Job summary

As an occupational therapist, youll help people who have difficulties carrying out day-to-day activities because of a disability, illness, trauma, ageing, and a range of long-term conditions.

Occupational Therapists (OTs) support people of all ages with problems resulting from physical, mental, social, or development difficulties. OTs provide interventions that help people find ways to continue with everyday activities that are important to them. This could involve learning new ways to do things or making changes to their environment to make things easier.

Main duties of the job

Occupational therapists are playing an increasing role in primary care. They enable people living with a range of health problems and chronic conditions to overcome the barriers so they can participate in everyday life and improve their health and wellbeing.

Occupational therapists assess the needs of people to do what they need and want to do (occupations). They develop plans with patients, so they can re-engage in everyday life, despite their health and social difficulties.

Through intervening early and taking a collaborative approach occupational therapists are able to address what matters to people. This can minimise crisis situations, prevent further deterioration and promote independence and social inclusion.

Based across our network of seven practices in Hammersmith & Fulham covering a population of 63,000, you will offer remote and face to face appointments. Key duties and responsibilities

- assess, plan, implement, and evaluate treatment plans, with an aim to increase patients productivity and self-care;

- work with patients through a shared-decision making approach to plan realistic, outcomes-focused goals;

- undertake both verbal and non-verbal communication methods to address the needs of patients that have communication difficulties;

- work in partnership with multi-disciplinary team colleagues, physiotherapists and social workers, alongside the patients' families, teachers, carers, and employers in treatment planning to aid rehabilitation;

- where appropriate, support the development of discharge and contingency plans with relevant professionals to arrange on-going care in residential, care home, hospital, and community settings;

Person Specification

Experience

Essential

  • experience in managing a designated caseload using theoretical and practical experience, completing assessments, planning, implementing interventions, and evaluating outcomes aligned to care plans
  • Demonstrated experience in operating at an advanced level of practice
  • experience of assessment and treatment delivery in long term conditions and active management
  • working knowledge of Microsoft and GP practice and prescribing data monitoring systems
  • ability to write comprehensive clinical notes, implement and evaluate care plans

Desirable

  • be aware of data protection (GDPR) and confidentiality issues particularly within a PCN

Communication

  • excellent interpersonal and organisational skills

Qualifications

Essential

  • has a BSc in or pre-reg MSc in Occupational Therapy under a training programme approved by the Royal College of Occupational Therapists is registered with the Health and Care Professions Council (HCPC)
  • is a registered member of the Health and Care Professionals Council
  • (HCPC)
  • is working towards the First Contact Practitioner roadmap training (Stage One) within six months of starting their PCN role.

Desirable

  • Pre-reg MSc in Occupational Therapy under a training programme approved by the College of OTs
  • Non-medical prescribing qualification
