Main duties of the job

They will have autonomous management of their own caseload of patients, providing a comprehensive, specialist clinical service for patients with often complex health and social care needs.

Please note the successful candidate will need a full driving licence and access to their own vehicle.

Job responsibilities

KEY ROLE & RESPONSIBILITIES

Job Purpose

To manage a caseload of service users with complex needs within the community, using a broad range of assessment and intervention skills.

To use specialist clinical skills to assess and deliver intervention to service users with complex needs within a variety of settings.

To contribute to the ongoing development of health and wellbeing provision for inclusion health population groups.

To work within a multi-disciplinary setting spanning medical, nursing, social care and allied health professionals as well as third-sector organisations.

To carry out work under the guiding principles of clinical governance and the Nursing and Midwifery Council (NMC).

Main Duties/Responsibilities

To manage, assess, plan and implement interventions for a caseload of complex service users, addressing service users needs.

To monitor, evaluate and modify treatment in order to measure progress and to ensure effective intervention.

To provide advice to service users and their carers to promote positive mental health and to ensure a consistent approach to care

To communicate effectively with service users and their carers to ensure understanding of health and social needs in collaboration with multi-disciplinary team members within the service.

In line with local guidelines, review and reflect on own practice and performance through effective use of professional and operational supervision and personal development review.

To contribute to the induction and education of staff, students, volunteers, and other workers/visitors who access the service.

To contribute to the measurement and evaluation of your work and current practice through the use of evidence-based practice projects, audit, research and outcome measures and implement any changes as required.

To undertake any other duties of a similar nature consistent with the responsibilities of this post in order to provide a quality service.

Provide crisis intervention to patients.

Refers patients as appropriate to other members of the multidisciplinary team, secondary care and to other statutory and voluntary organisations.

Initiates and carries out programmes of health screening.

Leadership

To deliver clinical supervision to staff identified in your area of responsibility.

To work with the objectives set for your team and monitor these within Bevans agreed performance management framework.

To ensure that the team adheres to legal, professional and organisational standards, policies and protocols.

To develop strong relationships with senior managers to promote effective cross-functional team working across Bevan that supports the achievement of the organisations objectives, standards and protocols.

To develop talent and diversity by ensuring all staff in your area of responsibility receive an annual performance and development review incorporating the development of a personal development plan.

To contribute towards the development and diversity of the team through sharing your clinical knowledge and expertise

Operate within information governance protocols and ensure business and patient confidentiality is maintained at all times.

Person Specification

Qualifications

Essential

  • Registered Nurse (Adult)
  • Degree in Nursing or other relevant subject (or equivalent degree level qualification)
  • Evidence of continuing professional development

Desirable

  • Mentor qualification
  • Independent prescribing

Experience

Essential

  • Minimum 2 year post-registration experience
  • Demonstrate the application of evidenced based practice
  • Able to demonstrate a depth and breadth of current issues and policies within the NHS and inclusion healthcare and have the ability to translate into practice
  • High level of clinical skills and competency relevant to the role
  • Understanding and experience of inter-agency working and adult/child safeguarding

Desirable

  • Public health experience
  • Experience of working with socially excluded groups (e.g. homeless, Gypsy/Traveller/Romany, asylum seekers)
  • Experience of teaching/mentoring students/groups
  • Experience in research, audit and bench-marking
  • Experience of clinical governance and risk assessment

Skills

Essential

  • Able to communicate highly complex, highly sensitive and/or contentious information, including where there are barriers to understanding
  • Demonstrate high level of self-awareness i.e. strengths and weaknesses, personal qualities and skills
  • IT skills
  • Able to communicate with a wide range of people at all levels of the organisation

  • Experience of working with interpreters
