Job summary
Rural Warwickshire North PCN are looking to recruit an
Enhanced Nurse to work closely with our practices, to enhance our team and
deliver quality care to a targeted cohort of patients.
It is a dynamic role to develop the frailty service across the PCN with stakeholders.
Please note this is not an Agenda for Change Contract, however we are able to offer the NHS Pension Scheme to the successful candidate(s).
Main duties of the job
This is a new role to our PCN and as such, the successful candidate will be contributory in developing and evolving the service as they progress.
To support GP Practices with the care of patients within our Care Homes, engaging the wider PCN teams where necessary.

We are a Primary Care Network (PCN) of six practices working together with a range of local providers, including primary care, community services, social care, and the voluntary sector, to offer more personalized, coordinated health and social care to their local populations.

The aims of a networks are to:
Protect, improve, and extend local General Practice through greater organisational scale to provide convenient high quality, comprehensive and integrated services.
Enhance the skills and flexibility of the General Practice workforce to meet population and service needs with programmes of continuing professional development.
Work and collaborate with others to innovate in service provision, building on new technologies and recognising lifestyle choices of service users in the way in which they wish to access care.

Job description
Job responsibilities
Work as part of the MDT within the PCN.
Working across 6 Practices and supporting the transformation of PCN projects.
Consults with patients, their family, and the multi-professional team to undertake assessments of patient need and devise and evaluate complex care plans.
Demonstrate initiative and are creative in finding solutions to problems.
Provide supervision to trainee nurse associate, nurse associates and general practice nurses that may be employed by the PCN.
Maintain quality patient records and use a database to store details of referrals and care pathways for specific patients.
Participate in clinical audits and research projects and implements changes as required, including the development, and updating of practice protocols / guidelines and procedures locally.
Develop effective working relationship with PCN member practices.
Develop and maintain productive relationships with key stakeholders; GPs, consultants, district nurses, social services, Trusts, voluntary sector etc.

Qualifications
Essential
- Registered Health Care Professional (eg HCPC/NMC)
- Relevant Masters Degree/180 credits at level 7 (Masters) in relevant area
- Works across the PCN, in services that enhance the core general practice offer
- Working at an enhanced level of practice as described in the Primary Care and General Practice Nursing Career and Core Capabilities Framework
Desirable
- Non-Medical Prescribing Qualification
Experience
Essential
- Experience and understanding of a wide range of approaches to the management of frail elderly patients
- Knowledge and understanding of clinical governance
- Understanding and knowledge of policy developments related to the delivery of primary care services including General Practice, the GMS/PMS contract, Clinical Governance, Quality & Outcomes Framework
- Demonstrate the ability to work in a highly demanding environment
- Ability to work independently and make autonomous clinical decision within own area of competence
- Evidence of continuing professional development
- High level communication skills, both written and verbal
- Ability to reflect on own practice
Desirable
- Awareness of current national and local agenda in health and social care, with a particular interest in Primary Care and Primary Care Networks
- Knowledge of audit / research and experience of analysing findings to improve practice
