Job summary

The post holder will be a pharmacist, who acts within their professional boundaries, supporting and working alongside practice teams within the Beckenham PCN and will be expected to move between sites of all 6 practices in the Beckenham PCN. They will be part of a team of 3 existing Clinical Pharmacists.

The post holder will ensure that the practice integrates with community and hospital pharmacy to help utilise skill mix, improve patient outcomes, ensure better access to healthcare and help manage workload.The role is pivotal to improving the quality of care and operational efficiencies so requires motivation and passion to deliver excellent service within general practice.

Main duties of the job

Post holder is an Independent Prescriber.

The post holder will work as part of a multi-disciplinary team in a patient facing role. The post holder will take responsibility for areas of chronic disease management within the Beckenham PCN and undertake clinical medication reviews to proactively manage patients with complex polypharmacy.

The post holder will coordinate and support practices within the PCN in the network QI requirements and also feedback the information to the Primary Care Network at meetings. They should also provide primary support to general practice staff with regards to prescription and medication queries. They will help support the repeat prescriptions system, deal with acute prescription requests, and medicines reconciliation on transfer of care and systems for safer prescribing, providing expertise in clinical medicines advice.

The post holder will provide clinical leadership on medicines optimisation, quality improvement and manage some aspects of the quality and outcomes framework and enhanced services.

About us

Beckenham Primary Care Network (PCN) comprises 6 local practices with a total population of around 60,000 registered patients. We are looking to expand the team to include pharmacists working across the PCN. This role will work closely with our expanding PCN team which includes community pharmacists, deiticians, nurse associate and social prescribing in addition to the core practice teams.

Knowledge, Skills and Experience Required

The post-holder will:

  • Have completed an undergraduate degree in pharmacy and registration with the General Pharmaceutical Council.
  • Have a minimum of 2 years experience as a pharmacist, demonstrated within a practice portfolio.
  • Have experience and awareness of the breadth of common acute and long-term conditions that are likely to be seen in a general medical practice.
  • Hold an independent prescribing qualification, or will be working towards an independent prescribing qualification.
  • Have the ability to plan, manage, monitor, advise and review general pharmaceutical care programmes for patients across core areas, including disease states/long term conditions.
  • Recognise priorities when problem-solving and identifies deviations from normal pattern and is able to refer to seniors or GPs when appropriate.
  • Be able to follow legal, ethical, professional and organisational policies/procedures and codes of conduct
  • Involve patients in decisions about prescribed medicines and supporting adherence as per NICE guidelines.

o Community and hospital pharmacy teams

o Hospital staff with responsibilities for prescribing and medicines optimisation

The post-holder will:

The post-holder will:

Health and Safety/Risk Management

  • The post-holder must comply at all times with the Practices Health and Safety policies, in particular by following agreed safe working procedures and reporting incidents using the organisations Incident Reporting System.
  • The post-holder will comply with the Data Protection Act (2018), General Data Protection Regulation (GDPR) (2016) and the Access to Health Records Act (1990).

Job Description Agreement

Person Specification

Qualifications

Essential

  • Mandatory registration with General Pharmaceutical Council (GPhC) which includes revalidation
  • Specialist knowledge acquired through postgraduate diploma level or equivalent training/experience
  • Independent prescriber or working towards/intent of gaining an independent prescribing qualification

Desirable

  • A member of or working towards Faculty membership of the Royal Pharmaceutical Society
  • Masters degree in pharmacy (MPharm) (or equivalent)
  • Specialist knowledge acquired through postgraduate diploma level or equivalent training/experience

Experience

Essential

  • Minimum of 2 years post-qualification experience.
  • In depth therapeutic and clinical knowledge and understanding of the principles of evidence-based healthcare.
  • An appreciation of the nature of GPs and general practices.
  • An appreciation of the nature of primary care prescribing, concepts of rational prescribing and strategies for improving prescribing.Excellent interpersonal, influencing and negotiating skills.
  • Excellent written and verbal communication skills.
  • Demonstrate the ability to communicate complex and sensitive information in an understandable form to a variety of audiences (e.g. patients).
  • Is able to plan, manage, monitor, advise and review general medicine optimisation issues in core areas for long term conditions.
  • Good IT skills.
  • Able to obtain and analyse complex technical information
