Job summary

The Mid Yorkshire Teaching NHS Trust is looking for Advanced Clinical Practitioners to work within the Division of Medicine in elderly care.

These roles are new within the Division and we are therefore looking for enthusiastic, motivated ACP's who have a desire to help change the face of elderly care, inspiring others to join an outward looking team who want to improve the care elderly patients have within our Trust .

The Elderly care team work on both the Pinderfields and Dewsbury sites and also have a virtual ward.

We have recently recruited trainee ACP's to our team and would love to have the experience of already trained ACP's within the team.

Main duties of the job

The primary aim of the role is to ensure that a high quality service is delivered to patients and their carers in a timely manner. The role is multidimensional and requires the post holder to integrate and fulfil all the identified dimensions. As an Advanced Clinical Practitioner, all activities provide opportunities for acting as a role model for staff

The Advanced Clinical Practitioner will act as an autonomous, accountable practitioner working on their own initiative and in collaboration with nursing, medical and allied health professionals to assess individual patient needs, initiate investigations, determine a differential diagnosis and initiate appropriate holistic evidence based treatment and care

The Advanced Clinical Practitioner will:

Manage their own caseload of patients who present with undifferentiated and undiagnosed illness.

Undertake audit within the clinical setting to develop an evidence base

Provide formal and informal teaching, support and clinical supervision to nursing staff and junior medical staff

Support the wider multidisciplinary team, undertaking independent comprehensive assessment and management of patients to incorporate physiological, psychological and pharmacological considerations, making multifaceted complex clinical decisions regarding patient management and clinical outcomes, including, where appropriate, the discharging patients from the service when treatment is complete/optimum outcome is achieved.

Key Responsibilities

  • To be competent in the assessment and treatment of patients presenting with a range of undifferentiated and undiagnosed illnesses underpinned by extensive knowledge of a range of chronic and acute conditions.
  • To be competent in providing an initial clinical diagnosis and in the decision to refer to an appropriate speciality.
  • To be competent in the ability to discharge patients following appropriate assessment and treatment.
  • To be competent in the initial immediate assessment and treatment of patients with a range of life threatening presentations such as cardiac arrest and acute major illness.
  • If not already an Independent Prescriber, will be expected to successfully complete the programme within a timely manner, in order to continue in the role. To be undertaken and completed within 1 year of qualifying.
  • To be able to see, treat and refer appropriately, a broad range of patients with acute and chronic illnesses as an autonomous practitioner.
  • Administration of medicines, e.g. in line with NMC Guidance and Trust Policy.
  • Demonstrate own personal development with self-directed learning and identifying and accessing internal and external development opportunities, as per individual advanced practice portfolio.
  • Autonomous decision making including judgments involving complex facts or situations which require the analysis, interpretation, and comparison of a range of options in relation to the care/ management needs of the patients on a day to day basis within the clinical environment.
  • Conduct physical assessments, examinations, and investigations to identify abnormalities and make diagnoses and administer a comprehensive and appropriate level of care.
  • Use of advanced clinical skills and autonomous judgement to prescribe drug treatments for patients following interpretation of clinical findings through requested bloods and investigations across several broad and varied clinical fields.
  • Use best practice, analyse evidence based practice, use of benchmarking to inform clinical practice.
  • Provide detailed and transparent reports of workload, outcomes, and recommendations for future developments; demonstrated in advanced practice portfolio this should include case based discussions, multi-source feedback, etc.
  • Be accountable for the direct delivery and implementation of clinical care on a day to day basis across the full spectrum of illnesses and therapy management.
  • Provide personal care plans to patients and develop, implement and evaluate care including independently prescribed medications, monitoring therapeutic programmes and providing health education and promotion.
  • Be proactive in developing new ways of working, such as ACP led initiatives to increase the quality of the patient journey and the ward efficiency.
  • Provide health education and advice to patients.

  • Be a safe and competent non-Medical Prescriber-providing responsible prescribing and adapting medication therapies to meet the needs of patients.

Person Specification

Qualifications

Essential

  • RGN Level 1 or HCPC registered AHP
  • Teaching Qualification
  • Masters level qualification in Advanced Clinical Practice with completed dissertation to achieve Band 8a
  • Qualified Non-Medical Prescriber
  • Recent and relevant continuous professional development

Desirable

  • ALS/APLS/NLS/ARNI provider

Experience

  • Extensive post registration clinical experience in an appropriate care setting Experience of working autonomously as an advanced clinical practitioner
