Job summary

We are currently seeking an Associate Nurses to Join our Easington South Durham Coast Community Nursing Team. The successful applicant will work across Easington South and surrounding areas from an office base in Peterlee. The team cover 8am until 9pm 7 days a week on a duty roster. The role involves providing holistic nursing care to adult patients in their own home or residential care setting. Sucessful applicants will be required to be Regulated by the NMC on the Nursing Associate part of the NMC register.

Main duties of the job

The Community nursing team provide both planned and reactive care including Urgent Community Response.

We provide hospital services from two acute sites - Darlington Memorial Hospital and University Hospital of North Durham. We have a centre for planned care in Bishop Auckland and provide care from community hospitals in Chester-le-Street, Shotley Bridge, Barnard Castle, Sedgefield and Weardale as well as over 80 other community based settings and providing care in patients' homes.

Job description

Job responsibilities

The sucessful candidate will provide a variety of nursing care to patients; including wound care, administration of medication, palliative care, management of bowel and catheter care. The role involves assessment of patient care needs, care planning, delivery of care and review. The sucessful candidate will be required to work independently as part of the wider Nursing team reporting back to senior staff on duty.

Person Specification

Qualifications

Essential

  • Level 5 Foundation degree specific to Nursing Associate practice
  • GCSEs Grade A-C in Maths and English or equivalent
  • NMC English language proficiency at the required level where appropriate.

Desirable

  • QCF in Care / Health and Social Care Level 2/3 or equivalent experience.
  • Care certificate.

Experience

Essential

  • Experience of working in teams under appropriate supervision as part of a multi-disciplinary team
  • Experience of providing and receiving complex, sensitive information
  • Able to recognize and work within the limits of competence and be responsible for own actions seeking support from the Registered Nurse when required
  • Able to communicate effectively (written, verbal and non-verbal communication) with patients/relatives and carers and all members of the multi-disciplinary team
  • Able to develop effective and appropriate relationships with people, their families, carers and colleagues
  • Understands the importance of following procedures and treatment plans
  • Able to organise and prioritise own delegated workload.

Desirable

  • Insight into how to evaluate own strengths and development needs, seeking advice where appropriate
  • Prepared to take part in reflective practice and clinical supervision activities

  • Registered Nursing Associate on the NMC register
  • Valid registration with the NMC

Special Skills & Knowledge

Essential

  • Understanding of the scope of the Nursing Associate in the context of the nursing and interdisciplinary team and the organisation and how the role may contribute to service developmentUnderstands and acts in line with NMC professional standards for practice contained within The Code.
  • Understand requirements for NMC professional revalidation
  • Knowledge of when to seek advice and escalate to the appropriate professional for expert help and advice
  • Understand requirements for NMC professional Revalidation
  • Understanding of the importance of following procedures and treatment plans
  • Able to support, supervise, assess and act as a role model to nursing associate students, other learners and health care support workers as required within the clinical setting
