PROPSPECT LPC are looking to recruit commissioning case managers via the bank who are available to support Greater Manchester LPC undertaking aspects of the case manager role particularly case manager quality reviews.

Main duties of the job

As a Case Manager, the post holder will work as part of a dynamic team in delivering an effectiveservice to ensure safe and effective care pathways are in place for all service users from their LATin specialised mental health care settings, within the NHS and Independent sectors.To provide clinical support to the specialist commissioners in relation to the defined population,this will include projected mapping of future service use, identification of users for future serviceprovision, planning in terms of future service needs.The role supports the business in driving transformation as well as value for money in planning,commissioning and service. The role is designed to build a combination of subject matter expertiseand technical skills to develop a strong service delivery.

1. To provide clinical assurance of the quality of the care provided by specialised services within the LAT.2. Improving quality and outcomes3. Supporting the team in the assurance of specialised services compliance to applicable specification and guidance.4. The post-holder will provide the link with Clinical Commissioning Groups and their commissioned services within the LAT to ensure there is a co-ordinated approach to careand discharge planning, which results in robust and effective care packages.5. Managing individual patient pathways to ensure high quality services and care planning outcomes are achieved6. To take action in respect of Safeguarding Policies and Procedures as required

Person Specification

Qualifications

Essential

  • Registered Mental Health/ Learning Disability Nurse or other registered health or social care professional.

Desirable

  • Possess a relevant additional professional qualification e.g., Forensic Psychiatry Diploma, Master's level, or equivalent level of experience of working at a senior level in specialist area.

Knowledge and Experience

Essential

  • Detailed knowledge of legal issues pertaining to the specialist service, including MHA 1983, after care under supervision, sex offender legislation, probation orders and life licence, Ministry of Justice procedures and child protection legislation.
  • Detailed operational knowledge of secure services, including risk assessment/management.
  • Extensive knowledge of specialist areas, acquired through post graduate diploma or equivalent experience or training plus further specialist knowledge or experience to master's level equivalent.
  • Evidence of post qualifying and continuing professional development.
  • Must have an understanding of the background to and aims of current healthcare policy and appreciate the implications of this on engagement.
  • Should have an appreciation of the relationship between the Department of Health and individual provider and commissioning organisations.
