This is a rare opportunity to help deliver our vision for an integrated system-wide approach to Frailty and Acute Medicine with one of the country's best performing frailty services (Frailty CQUIN 2023).

The Frailty team is a large multi-disciplinary team based in the Emergency Department and which runs an Acute Frailty Assessment unit delivering same day emergency care for patients with frailty. The team comprises of 1.7 frailty consultants, 1 registrar, 2 lead nurses, 5 clinical nurse specialists and 4 Physicians Associates. The service provides diagnosis, treatment, and facilitates early discharge of frail patients and provides interface with frailty virtual ward.

The post holder will work across our Acute Medicine department on the Medical Assessment Unit (MAU), the Short Stay Unit (SSU) and the Ambulatory Care Unit (ACU -SDEC). The MAU sees, assesses and treats patients in our acute unselected medical take. Ambulatory care is overseen by a consultant and staffed by specialty doctors and by advanced nurse practitioners. The Short Stay Unit receives medical patients likely to be discharged within 72 hours of admission and achieves a high turnover of patients per day.

Main duties of the job

  • Provide a high quality, safe and compassionate care to the acutely ill medical patients admitted to our hospital and provide them with the best possible experience during their admission.
  • Active participation in Trust Clinical Governance activities, including audit, mortality review and appraisal process. The appointee will ensure data collection and processing undertaken is consistently accurate, timely and complies with the Trust Data Quality Policy and Information Governance procedures.
  • Teaching, training and supervision of doctors, nurses and students in order to promote a positive and supportive atmosphere.
  • Responsible for keeping a record of Continuous Professional Development (CPD), including Trust Mandatory training process.
  • Will be responsible for engaging in regular appraisal processes for revalidation
  • The provision of a first-class clinical service.
  • Effective leadership to all staff engaged in the specialty.
  • Sustaining and developing teaching and research.
  • Undertaking all work in accordance with the Trust's procedures and operating policies
  • Conducting clinical practice in accordance with contractual requirements and within the parameters of the Trust's service plan
  • Maintaining the confidence of business plans and development strategies formulated for the speciality, the Care Group of the Trust.

  • Lead the frailty team whilst on direct clinical care.
  • Attend Frailty team meetings.
  • Attend service improvement meetings.
  • Attend monthly ICS meetings, engage with falls hub meetings and North Place meetings.
  • Attend UEC governance meetings.
  • Carry out annual reviews of Physicians Associates whom you are supervising.
  • Support education programme for junior staff in UEC and frailty team.
  • Work closely with the Emergency department team.
  • Work closely with the Acute medicine team.
  • Partaking in the appointment of new members of the frailty team.
  • Take responsibility for own continuing professional development and take part in continuing medical education activities, in accordance with any relevant standards for maintaining GMC licence to practice and membership of any relevant professional body.

Person Specification

Qualifications

Essential

  • Registered medical qualification. MRCP or equivalent.
  • Full registration with GMC and Name on Specialist register from GIM on date of taking up appointment.
  • Experience in and knowledge of Geriatric Medicine.

Desirable

  • Higher degree or diploma (e.g. MD, PhD).
  • Name on Specialist Register for Geriatric Medicine or Acute Medicine.
