Job summary

Arch is an outstanding healthcare provider delivering primary care to homeless and vulnerably housed patients in Brighton and Hove. In addition to a specialist GP surgery, we also provide outreach services into the Royal Sussex County Hospital and a variety of community contexts which serve people who are without secure accommodation. We are a committed and experienced team with a passion for improving the health and well-being of people who are homeless or vulnerable.

We are looking to appoint a part-time Health Engagement Worker to assist with the delivery of an enhanced outreach service which delivers healthcare on the streets, in day centres, hostels and emergency accommodation. Movement between settings will largely be conducted on ebikes to facilitate case-finding. This project has been running for 18 months, and so requires staff to demonstrate a high level of flexibility, drive and ambition, in order to achieve the benefits which we anticipate for our patients. You'll work closely with the Outreach Team Coordinator who is an experienced clinician and other staff from Arch to ensure the success of the project.

This is a fixed term position for 18 months (maternity cover). Interviews will be held on Friday 12 July.

Job responsibilities

Job purpose

  • Patient focus: Facilitating patient engagement through face to face and telephone interactions, providing follow-up between sessions to support patient recovery.
  • Project focus: Providing logistical support to the delivery of the project including all practical and administrative aspects.
  • Partner focus: Working closely with the Project Coordinator to deliver clinical outreach in Brighton & Hove, ensuring strong integration into the wider homeless health network.

Core duties

  • Supporting individual patients to engage with the service, this will include face to face and telephone interactions.
  • Advocacy in registering patients for medical and support services and assisting them in addressing all aspects of physical and mental health or substance addiction needs, as well as advocating for them to get access to housing and social security benefits.
  • Providing logistical support to the project, for example collecting medications or accompanying patients to follow-up appointments.
  • Designing and delivering health promotion sessions, for example to improve vaccine or cervical screening uptake.
  • Liaising with partners to arrange and plan outreach sessions.
  • Advocacy for patients.
  • Building partnerships with other services.
  • Use of IT systems for record keeping.
  • Engaging in learning and development exercises relating to the project as it develops.
  • Working with the wider Arch team.
  • Contributing to report writing.
  • Working to support the Outreach Team Coordinator with all aspects of the project

  • Knowledge of assessment, support planning and interventions that help clients experiencing homelessness or housing vulnerability to become and stay healthy.
  • Knowledge of the challenges and solutions to working with a client group who may be experiencing one or more of the following: poor health (physical and mental), substance misuse and homelessness.
  • An understanding of health and safety policies and procedures that aim to keep staff and clients safe.
  • Care, empathy and skilled in dealing with emotionally vulnerable service users.
  • Excellent ability to engage with service users in an empowering manner.
  • Effective communication; written and oral skills.
  • Effective administrative and IT skills.
  • Excellent organisational skills.
  • Ability to ride a bike.

Desirable

  • Full driving licence.

Experience

Essential

  • Experience of working with vulnerable adults (those experiencing homelessness or housing vulnerability, or a comparable client group).

Desirable

  • Experience of working within a health environment and/or with drug and alcohol dependent adults or those experiencing poor mental health.
