Job summary

We are recruiting Advanced Clinical Practitioners (ACP) from a variety of professional backgrounds including nurses, pharmacists, paramedics and physiotherapists.

The ACP role within the UTC/ED is a new post and offers an excellent opportunity for experienced Practitioners to be involved in delivering person centred care while working as an autonomous Practitioner.

This is the perfect opportunity to be part of a diverse team providing assessment, diagnostics, diagnosis, treatment, advice and referrals

As an Advanced Care Practitioner, you will be considered a key person in day-to-day clinical leadership and expertise within the department(s)

Hours of work: 37.5 hours per week. The shift pattern is required to be flexible to meet the needs of the service over a 24-hour period.

If you are experienced with a master's level award or equivalent and keen to take on an extended role within an Urgent and Emergency Care environment, we would welcome your application.

We welcome applications from those who currently hold a UK NMC registration. If you currently hold an overseas nursing qualification and require sponsorship we are not currently direct hiring Internationally Qualified Nurses.

Main duties of the job

You will be working autonomously within your sphere of competency as a clinician in a diverse multi skilled team that includes Emergency Medicine Physicians, General Practitioners, Emergency Nurse Practitioners, EM Nurses and administrative staff.

The post holder will deliver care to patients attending the UTC/ED with undifferentiated diagnoses covering all aspects of minor and major illness and injury.

They will have the ability to adapt to changing levels of activity and acuity within the UTC/Emergency Department in an autonomous role, exercising advanced practice skills, knowledge and clinical expertise.

The post holder will have proven clinical skills, while demonstrating excellent leadership and communication skills.

The post holder will have experience of working effectively across professional boundaries.

Job responsibilities

As an Advanced Care Practitioner, you will be considered a key person in day-to-day clinical leadership and expertise within the department. The Advanced Care Practitioner will demonstrate highly developed specialist knowledge and skills in the management and care of the Urgent/Emergency patient.

You will work autonomously within your sphere of competency as a clinician in a diverse multi skilled team which includes General Practitioners, Emergency Nurse Practitioners and administrative staff. They will ensure patients are fully informed regarding their proposed treatment, provide appropriate information and support liaising with appropriate colleagues to ensure a seamless service for the patients.

The Advanced Care Practitioner will formulate clinical decisions and treatment plans to manage the urgent and/or emergency patient illness and injury. The Advanced Care Practitioner will demonstrate specialist knowledge and skills in the management of care of the urgent and/or emergency patient

They will carry out holistic needs assessments of patients, initiating interventions and referring to members of the multidisciplinary teams as appropriate. They will exercise advanced clinical expertise, levels of judgement for the development and review of care protocols and training for staff within their area of specialism.

Audit and monitoring of standards of patient care, using the results to influence practice, thereby continually improving evidence-based practice and delivery6 of patient focused care. They will be required to develop and maintain links with local and national strategic organisations.

Person Specification

Experience

Essential

  • Professional Registration with either NMC, HCPC or GPhC.
  • MSc Advanced Practice or equivalent / other MSc mapped to the four pillars of advanced practice.
  • Able to demonstrate competence at an advanced practice level through Royal College or HEE Credentialing
  • Evidence of Continuing Professional Development.

Desirable

  • Relevant NHS Leadership qualification
  • Significant demonstratable experience of working as an autonomous practitioner in within the relevant specialty such as Geriatric Medicine, Palliative Care or Intermediate care/community rehab services
