PLEASE NOTE: This position is for 4 sessions. Full day Tuesday and Friday .

-To provide a full range of medical services to the practice patients-

To work as an effective member of the practice team, working within practice guidelines and procedures-

To provide high quality primary care general medical practitioner services to the practice population

- To work as part of a multidisciplinary team with other members of the practice and community teams.

Main duties of the job

Provide a full range of medical services as appropriate for the practice patients, within the agreed surgery timetable

To provide on call service for the Practice on a rotational basis

To take a reasonable portion of clinical workload within practice time including responsibility for booked surgeries, urgent patients and visit those patients who are unable to attend the surgery as appropriate.

To speak on the telephone to patients who require medical advice

To be actively involved and responsible for the continuing care of patients, including correspondence and results

To provide the on-call service for the practice when required

To be aware of the requirements within the Quality and Outcomes framework and to work towards maximising the achievement within the practice by recording information appropriately.

Job responsibilities

Provide a full range of medical services as appropriate for the practice patients, within the agreed surgery timetable

To provide on call service for the Practice on a rotational basis

Maintaining the highest clinical standards at all times and in accordance with accepted good medical practice and the various clinical policies of the practice/centre.

In accordance with the practice timetable as agreed, the post-holder will make him/herself available to undertake a variety of duties as expected from a GP including surgery consultations, telephone (or other) consultations and queries, visiting patients at home, checking and signing repeat prescriptions and dealing with queries, paperwork and correspondence in a timely fashion

Making professional, autonomous decisions in relation to presenting problems, whether self-referred or referred from other healthcare workers

Assessing the healthcare needs of patients with undifferentiated and undiagnosed problems

Screening patients for disease risk factors and early signs of illness in consultation with patients and in line with evidence-based best clinical practice developing treatment and any associated care plans

Recording clear and contemporaneous consultation notes to agreed standards

Championing the practice clinical governance programme for the provision of safe, high quality patient care

Compiling and issuing computer-generated acute and repeat prescriptions (avoiding hand-written prescriptions whenever possible)

Prescribing in accordance with local prescribing formulary (and generically) whenever this is clinically appropriate

To deal with queries relating to repeat prescriptions and ensure adequate oversight and safety in the management of these.

Supporting the achievement of QOF

Supporting other members of the practice clinical team providing clinical supervision as required

Ensuring that when referrals are made for patients to see other medical service providers, adequate information is given to the personnel to whom the patient has been referred for proper assessment to be made

Person Specification

Experience

Essential

  • - Full and current unrestricted GMC (UK) registration on the GP Register MRCGP or working towards it
  • Be on a Medical Performers List of a Primary Care Trust in England
  • Hold current medical indemnity from a recognised medical defense organisation.
  • Excellent command of the English Language both verbal and written.
  • GP Vocational or additional experience in general practice. Commitment to professional development Knowledge of the NHS. Understand the importance of evidence based practice and clinical effectiveness.
  • Understanding of and adherence to clinical governance.
  • Ability to make good clear concise medical notes, both computerised and manual.
  • Experience of Microsoft Office applications Experience of Practice Based computer systems
  • The Practice is an EMIS web practice and experience of this system or of System One is essential.
  • Excellent interpersonal and communication skills. A solutions focused approach.
  • Ability to listen and empathise Ability to undertake responsibility.
  • Able to work as part of a multi-disciplinary team.
  • Flexible and a good time keeper Good organisational and time management skills.
  • Able to work under pressure and within an ever changing environment. Commitment to continuing professional development Hold a full and valid driving licence, own a car and be prepared to use it for business mileage.
  • Experience of all GP Clinical systems.
  • Ability to perfrom Home Visits for Housebound patients.
  • Ability to manage and facilitate change.
