SWL ICB - All Age Continuing Care teams have recently been organised into 3 teams each covering 2 local authorities and are responsible for the delivery of ICB statutory responsibilities of the National Framework for NHS Continuing Healthcare (CHC) and NHS Funded Nursing Care (FNC) [ revised July 2022]. This post is an exciting opportunity for the successful candidate to build on existing management skills and knowledge of CHC to deliver a first class CHC service and be part of the change in service to reflect the All Aged Continuing Care Service required by NHSE.

This post reports to the Deputy Director for CHC at Place and will be expected to deputise for them as when necessary including representing the SWLICB locally engaging with parties statutory and non-statutory and at NHSE.

The successful candidate requires a registered professional qualification in health or social care with ve l excellent communication, previous experience of working within and managing a CHC clinical team, driven and motivated for the challenge of moving the team forward to deliver an All Aged Continuing care service. The SWLICB requires the service model to deliver a high-quality service that is robust and framework compliant.

Main duties of the job

This position is a senior leadership role responsible for overseeing and managing the operational delivery of the assessment, decision making and commissioning of care packages for those assessed as eligible to receive Funded NHS Continuing Healthcare and FNC. The teams provide a full end to end service for all adults aged over 18 years clients who are eligible for CHC funding, including the delivery of care throughAQP framework for standard nursing, domiciliary care or Personal Health Budget.

The post holder will be responsible for the operational running of the CHC place teams, providing clinical leadership, accountable for performance meeting KPI's and ensuring high quality delivery of the CHC service that supports the ICB in meeting the statutory requirements of the national framework that delivers on quality and meets the KPI's for delivery and finical performance set by NHSE.

This post supports the ICB to deliver a service that is equitable across of clients groups, delivers a high quality service that places the individual central to decision making and that the commissioned care services meet the individual's assessed care needs, delivers on quality , health outcomes and is cost effective.

Person Specification

Education / Qualifications

Essential

  • Registered health care professional with extensive management and clinical experience
  • Educated to Master's level or equivalent level of experience of working at a senior level in specialist area. 2
  • Evidence of post qualifying and continuing professional development
  • Member of relevant professional body (NMC)

Knowledge and Experience

Essential

  • Expert knowledge of specialist areas - the National Framework for NHS Continuing Healthcare and Personal Health Budgets
  • Demonstrable experience of managing a Continuing Healthcare team
  • Proven understanding and experience of individual placements including NHS Continuing Healthcare
  • Excellent understanding of specialist areas (Mental Health Act, Section 117 protocol, Children's Continuing Care, PHBs and Neuro Rehab),
  • Expert knowledge of responsible commissioner guidance
  • Demonstrable experience of explaining CHC eligibility decisions to patients, relatives and local authority professionals
  • Experience of presenting and sitting on NHS England IRP panels
  • Should have a good understanding of the relationship between the Department of Health, NHS England and individual provider and commissioning organisations
  • Expert complex case management skills
  • Ability to manage challenging and complex situations proactively
  • Experience of managing, motivating and supporting teams through change programmes

Skills and Abilities

Essential

  • Highly developed communication skills for delivering key messages to a range of stakeholders both internal and external (including outside the NHS) to the organisation, some at very senior level.
  • Good presentational skills for conveying complex concepts.
  • Ability to use informed persuasion to influence others.
  • Ability to identify risks, anticipate issues and create solutions and to resolve problems in relation to project or service delivery.
  • Ability to understand a broad range of complex information quickly and making decisions where opinions differ/no obvious solution.
  • Demonstrable evidence of planning and delivering programmes and projects and services on time.
