Staff Nurse - Urgent Treatment Centre

To assess, plan, implement, evaluate and provide high quality patient care, supporting in the provision of clinical expertise, while working effectively as a member of the Multidisciplinary Team.

Completing initial assessments on patients presenting to the Urgent Treatment Centre, providing treatment and medications, to patients presenting with minor injuries and illness.

To manage a caseloads of patient within defined clinical areas.

To always act in a manner consistent with the Trust Values and NMC Code of Professional Conduct, carrying out his/her duties in accordance with Trust policies and procedures.

To lead, support, supervise and teach staff where appropriate.

The UTC is part of the Urgent Care family, which also includes the Emergency Department, Acute and Integrated Assessment Unit and Same Day Emergency Care.

SECTION 1- MAIN DUTIES & RESPONSIBILITIES:

Clinical Responsibilities

  • The post holder must demonstrate competence in a wide range of expanded roles including: Venepuncture, cannulation, ECG recording & basic interpretation, plastering, requesting and obtaining pathology samples, utilising PGDs, requesting x-rays.
  • Assess, Plan, implement and evaluate individualised patient care through appropriate frameworks, including safe effective arrangement for discharge and transfers of care, underpinned by Trust Values.
  • The post holder must be able to demonstrate a high level of clinical decision making skills.
  • Excellent communication skills are required to ensure that patients, relatives and the multi-disciplinary team are able to understand the often complex needs of the patients.
  • To carry out responsibilities in such a way to minimise risk of harm to children, young people and vulnerable adults and to promote their welfare in line with the legal framework and Trust policies and procedures.
  • Maintain effective communication within the team by ensuring that information is transmitted at handover and is documented either in the notes, appropriate folders or on the computer system as deemed necessary.
  • All documentation relating to a patients episode within the department is to be recorded accurately in the patients notes/computer system for future referencing as required.
  • Assist in maintaining a calm, efficient and cost effective service.
  • Assist in Co-ordinate/facilitate new staffs orientation into the department.
  • Undertake individual responsibilities as delegated to the post holder.

PROFESSIONAL RESPONSIBILITIES:

  1. Maintain a professional behaviour at all times in line with Trust Values
  2. Practice in accordance with and adhered to his/her professional responsibilities under NMC Code of Professional Conduct and other NMC guidelines
  3. Participate in the setting of standards for nursing practice and for care involving the Multidisciplinary Team
  4. To participate in and support the quality governance agenda, including undertaking audit activity, complaints and incident investigations.
  5. To maintain clinical practice standards and knowledge, and act as a professional role model for all nursing staff.
  6. Use own clinical judgement to initiate appropriate nursing interventions prior to medical input.
  7. Maintain accurate, current and contemporaneous records and ensure that all records relating to patient care are clear, concise and legible in accordance with the NMC Standards for Records and Record Keeping.
  8. Ensure adherence to the Trust Medicines Policy and the NMC Standards for the Administration of Medicines.
  9. Assist in the leadership of specific areas of practice developments in accordance with local and national policies and strategies to improve patient care.
  10. Actively assess and manage risk within the clinical area in accordance with Health and Safety legislation and risk management policy.
  11. Work with information governance for information sharing.
  12. To maintain continuous professional registration with the NMC throughout the duration of employment with the Trust.

EDUCATION AND TRAINING:

  1. Act as a clinical supervisor to junior clinical staff completing appraisals and agreeing personal development plans ensuring systems are in place for staff to be able to reach their objectives.
  2. Promote clinical innovations and encourage evidence-based practices within the department.
  3. Assist in the developments of components for a teaching program for use within the department.

ADMINISTRATIVE/MANAGERIAL:

  1. Work within clinical governance systems and ensure that care is delivered within an appropriate model within the Urgent Care area.
  2. Ensure adherence to all Trust policies.
  3. Develop and implement protocols and or policies within own specialist area.
  4. Effectively contribute to the achievement of Trust targets.
  5. Support the Team Leader and Matron for Urgent Care in effective resource management. Participate in the identification of cost improvement initiatives with Team Leaders and Matron.

HUMAN RESOURCES MANAGEMENT:

  1. Support effective intra and inter departmental communication.
  2. Provide competent and consistent leadership to the department team.
  1. Undertake appraisals for junior colleagues and ensure PDPs are in place.
  2. Participate in training and development for all staff, ensuring that accurate records are maintained.
  3. Be actively involved in the induction and mentorship for newly deployed and newly qualified staff.
  4. Ensure that educational objectives of student nurses and other learners are met and the progress of each student/learner is monitored.
  5. Participate in clinical supervision in accordance with Trust policy.

SECTION 2 - KNOWLEDGE AND SKILLS

  1. Possess evidenced based clinical knowledge appropriate to the area.
  2. Utilise own clinical expertise and knowledge to create and promote an environment conducive to meeting the educational and developmental needs of all clinical staff.

SECTION 3 - EFFORT AND ENVIRONMENT

  1. Partake frequently in tasks requiring physical effort for long periods
  2. Regularly deal with bodily fluids and soiled linen within a span of duty.
  3. Manage and participate in distressing unpredictable situations frequently within a span of duty.
  4. Maintain contemporaneous patient records at all times
  5. Concentrate for long periods and respond to unpredictable patient and staff demands.
  6. Provide and receive complex information that will require empathy, tact, reassurance and communicate in situations where there maybe barriers to understanding.
  7. Work with patients with mental health needs and liaise with relevant speciality in relation to clinical management.
