We have an exciting NEW opportunity for you to join one of our forward thinking and innovative integrated 0-19 Universal team in Ealing at Central London Community Health Trust.

Central London Community Healthcare Trust (CLCH) has an exciting opportunity available for passionate and committed School Nurse to work across Ealing as a Public Health Quality Fellow. We are looking for candidates with excellent clinical and communication skills to join our School Nursing team contributing to delivery of 5- 19 Healthy Child Programme. You will help make a tangible difference to peoples' lives and enjoy the very best of London life on your doorstep.

Main duties of the job

The School Nursing Team is seeking a dynamic individual with a strong public health focus who is fully committed to improving the health and wellbeing of our diverse school aged population. You will be a highly motivated individual with an interest in proactively managing health promotion initiatives.

The SCPHN Trained Public Health Quality Fellow is a new initiative to CLCH; leading, influencing, and delivering a comprehensive School Nursing service in line with the Healthy Child Programme and local service specifications for School Nursing that meets the public health needs of local school communities; contributing to the delivery of Working Together to Safeguard Children (2018), including improving health and life chances for children, young people and families and safeguarding those at risk and to deliver the role in accordance with CLCH Trust values and behaviours.

You will undertake the School Nursing role for 80% of your clinical time. 20% of your role will be dedicated to working within a structured 6-9-month work-based programme working under the supervision of the Public Health Nurse Consultant. You will identify, design, and manage a project focusing on health at a population level, looking at ways to make school communities and environments healthier, reducing ill-health and tackling health inequalities.

Job responsibilities

  • Focus on health at a population level, looking at ways to make school communities and environments healthier, reducing ill-health and tackling health inequalities.
  • Measure, monitor and report population health and wellbeing; health needs, risks, and inequalities; for a specific chosen area of your work.
  • Work with the Trust Public Health team and existing childrens services to promote population and school community health and wellbeing, addressing the wider determinants of health and health inequalities.

Audit, evaluate and re-design services and interventions for a specific chosen area of your work to improve health outcomes and reduce health inequalities using Quality Improvement (QI) methodology.

  • Be trained in Bronze, then Silver QI to undertake their project.
  • Work with, and through, policies and strategies to improve health outcomes and reduce health inequalities.
  • Work collaboratively across agencies and boundaries to improve health outcomes and reduce health inequalities for a specific chosen area of your work.
  • Provide leadership to drive improvement in health outcomes and the reduction of health inequalities.

Education/Qualification

Essential

  • Registered Nurse
  • Registered School Nurse (SCPHN)
  • Current NMC registration
  • Evidence of continuing post-registration education and training
  • Practice assessor/supervisor training or working towards

Experience

Essential

  • School nurse experience
  • Be able to co-ordinate health promotion initiatives in line with local and national health improvement targets
  • Experience of working collaboratively as part of a multidisciplinary team and inter-agency working
  • Experience of organising and managing own workload and that of others
  • Experience of assessing, planning, and implementing programmes of care and writing care plans
  • Experience in providing practice-based learning support to others.
  • Experience of evidence-based care.
