Job summary

An exciting opportunity has arisen for a Band 5 occupational therapist to join the Tameside Adult Community Learning Disability Care Hub on a permanent full time basis.

This post would be suitable for a newly qualified Occupational Therapist.

The successful candidate will be a highly motivated, person centred individual with excellent communication skills who will be joining our well established Community Learning Disability Team.

We aim to provide high quality, community based, integrated services for adults with learning disabilities. As such, we are seeking an individual with a passion for working within and improving learning disability services.

Main duties of the job

We are seeking an enthusiastic and well-motivated Occupational Therapist to work as part of our multi-disciplinary community team providing services to adults with Learning Disabilities. The team covers the Tameside and Glossop area.

The post holder will hold a caseload and work autonomously. They will use a variety of approaches with service users who have complex needs including autism. Previous knowledge and experience of working with people with learning disabilities would be an advantage.

The community team consists of senior OT, physiotherapists, nurses, psychologists, consultant psychiatrist, speech and language therapy, Autism diagnostic team and Learning Disability and Mental Health Liaison Practitioners. The post holder will visit clients in their own homes, day services, residential care homes or other community settings.

Job responsibilities

To undertake occupational therapy assessments addressing occupational performance, strengths and skills deficits, enabling the service user in areas of self-care, productivity and leisure.To collaboratively identify the goals of occupational therapy intervention with the service user as part of the overall care plan.To plan and implement individual and/or group evidence-based interventions, in collaboration with the service user, using graded activity to achieve therapeutic goals.To monitor and evaluate treatment in order to measure progress and ensure effectiveness of the intervention.To provide support and education to service users and carers regarding aspects of occupational therapy. To provide supervision for Occupational therapy students and apprentices.To attend appropriate case conferences, clinics, referral and review meetings, contributing to decisions with regard to service user care plans.To assess and actively manage individual activity and environmental risk to ensure the safety of the service user, self, colleagues and wider public, whilst recognising the right of the service user to take positive risks in promoting their recovery. To ensure that up to date written and electronic records are maintained in accordance with Professional and Trust standards.To take responsibility for prioritising own clinical case load.

Person Specification

education and qualifications

Essential

  • Diploma/degree in occupational therapy
  • HCPC registration

Desirable

  • Experience working in LD as a qualified OT/apprentice or student

Experience

  • Experience working within an MDT/community setting.

  • Relevant clinical experience

Knowledge

Essential

  • Good understanding of the role of an Occupational Therapist in a learning disability setting
  • Good understanding of MOHO theory and assessment.
  • Good understanding of risk assessment.
