Job summary
This
is a new and innovative post with a real opportunity to make a difference and
develop a genuinely primary care led dietitian service. We are looking for a self-motivated,
committed dietitian to develop and provide a quality dietetic service.
Main duties of the job
Assess, diagnose, triage, and manage
patients, taking responsibility for the management of a caseload.
Develop and make use of your full
scope of clinical practice, including skills relating to supplementary
prescribing and investigation when trained to do so.
Manage complex interactions, including
working with patients with psychosocial and mental health needs, referring to
social prescribing when appropriate.
Deliver programmes of supported
patient self-management, in ways that facilitate behavioural change, optimise
individuals' health, nutrition, fulfilment of personal goals and independence,
and that minimise the need for pharmacological interventions.
Provide care which is tailored to the
individual needs.
Make prevention possible in primary
care with dietetic interventions for lipid lowering and diabetes prevention and
remission.
Support patients to set their own
goals and be confident in their approach to self-manage their conditions where
appropriate e.g. diabetes, high cholesterol & cardiovascular disease.
Liaises with ICB colleagues including
commissioners, to ensure consistency of patient care and benefit to GP
practices.

Job description
Job responsibilities
The post-holder will bea qualified autonomous clinical practitioners working with our GP practices who can assess, diagnose, treat, and manage
dietetic problems and undifferentiated conditions, and where appropriate, to
discharge a person without a medical referral. First Contact Dietitians are a key multidisciplinary
team member in primary care and support reducing GP workload. FCDs working in
this role can be accessed directly by self-referral or staff in GP practices
can direct patients to them to establish a rapid and accurate management plan
to streamline pathways of care. FCDs will also be part of the wider PCN team
workforce and will have regular clinical supervision.

Qualifications
Essential
- Undergraduate or postgraduate HCPC approved dietetic programme
- Health & Care Professions Council (HCPC) registration
Desirable
- Completion of some level 7 MSc learning
- BSc or pre-reg MSc in Dietetics under a training programme approved by the British Dietetic Association (BDA)
Experience
Essential
- Clinical experience of triage and care planning
- Ability to evidence a sound understanding of the NHS principles and values
- Ability to analyse and interpret complex/ often incomplete information, preempt and evaluate issues, and recommend and appropriate course of action to address the issues
- Minimum 3 years of clinical experience in dietetics
Desirable
- Minimum of 3 years post graduate dietetic experience in a clinical areas e.g. community dietetics, outpatients, GP clinics, home visits
- Experience of working within a primary care setting
- Previous experience of working in a GP practice
- Previous clinical experience of first contact triage clinical work; even if in the private sector or freelance
- Previous experience of using EMIS / SystmOne
- Evidence of working across organisational boundaries within health and social care
- Independent thinker with good judgement, problem-solving and analytical skills
