Job summary

Due to a secondment, the Heart Failure Clinical Nurse Specialist Team are looking for an efficient and responsive administrator to join them in contributing to the delivery of high quality care for their patients.

The Heart Failure Team provide cross site specialist care to both inpatients and outpatients who have been diagnosed with Heart Failure. It is a growing team with an expanding service and therefore the successful candidate should be flexible, have excellent communication skills and be able to work well as part of a team.

In addition to general administration duties and clinic bookings, there will also be a requirement to collate and input a wide range of data for use in the National Heart Failure Audit, in a timely manner.

Data input skills would be advantageous, and you will need to be responsive to the needs of the service. You will be a point of contact for patients, specialty teams and other departments for enquiries relating to Heart Failure reviews.

The successful candidate will be based within the Heart Failure Office at Peterborough City Hospital, although as the service expands then there may be a requirement for occasional cross site travel within the organisation.

Main duties of the job

  • Utilising Microsoft Office packages including Excel, Word and E-mails and utilising other databases as required (e.g. for NICOR) to provide an efficient service for the team.
  • Responding to enquiries from patients/departments/other heart failure services (email, letter and telephone) ensuring good communication and appropriate response.
  • Providing efficient and comprehensive administrative support required to meet the needs of the Heart Failure Nursing Service
  • Ensuring appropriate heart failure clinic follow up.
  • Responding to enquiries from patients/other departments (email, telephone, letter) ensuring good communication and appropriate response.
  • Collecting and collating a wide range of data during a patients' admission for heart failure, for use in national and local audit/reports.
  • Ensuring that patient referrals out to community heart failure services (& other services) are managed efficiently and in a timely manner.
  • To undergo training as necessary to meet the needs of new developments within the service.

MAIN RESPONSIBILITIES

TEAMWORK

  • The post holder will:
  • Maintain good working relationships with colleagues within the HF team and the cardiology nursing specialist teams and in other Trust departments.
  • Communicate in an appropriate manner with all staff within cardiology, other departments across the Trust and bodies external to the Trust.
  • Work flexibly and liaise with cardiac rehabilitation administrator to maintain and ensure smooth running of the HF and CR services (gaining additional skills as required).
  • Deal with any problems in a constructive manner, communicating with relevant staff as necessary.
  • Advise the HF Team Leader of any points of concern relating to the administration of the service.
  • Attend meetings when relevant.
  • Be aware of workload and pressures within the team/other specialist nursing team enabling support and encouragement given upholding Trust values and behaviour.
  • To ensure adequate cover is in place during absence by liaising with the Cardiac Rehabilitation administrator

COMMUNICATION

ADMINISTRATION PROCESSES

Audit

  • To support the heart failure nursing team in the collection of data for the National Institute for Cardiovascular Outcomes Research (NICOR) -Heart Failure audit.
  • To input data on to the NICOR database in a timely and efficient manner.
  • To maintain collection of in-hospital data (as directed) for use in local audits.
  • To assist the team in compiling datasets required for audit/service improvement when require

Clinics

  • To ensure any actions detailed in clinic have been completed i.e. booking of follow up appointments
  • To keep an accurate account outstanding appointments, patient cancellation/rebooks.
  • To liaise with cardiology secretaries, outpatient booking team and patients answering any queries regarding heart failure nurse clinics.
  • To input Cardiac Rehab clinic outcomes, make new/follow up clinic appointments and update spreadsheets as directed, when covering Cardiac rehabilitation administrator

General Administrative duties:

  • Ensure referrals to the community heart failure nurses are actioned in a timely manner.
  • Supporting the Heart Failure Nurses by responding to generic emails/telephone/letter queries and highlighting/prioritising action required to the nursing team.
  • Prepare MDT list weekly and input outcomes onto patient timeline.
  • Accurate filing of information office based filing/Evolve
  • Maintaining a stock of patient leaflets/office supplies
  • Take notes at heart failure meetings and produce typed minutes
  • To undertake other administration tasks as required by HF nursing team.

Person Specification

Qualifications

Essential

  • Educated to GCSE level in English Language and Maths or equivalent
  • ECDL or equivalent experience

Desirable

  • NVQ 3 Business and Administration or equivalent experience

Experience

Essential

  • Previous experience and/or can demonstrate an overall understanding patient pathways
  • Systems administration experience.
  • Experience of supervision of Administration/Secretarial Staff

Desirable

  • Experience of working in the NHS in an administration role
  • Experience of using PAS, databases, e-track Anglia Ice, MS office and Excel Spreadsheets

  • Can demonstrate good standards of written and verbal communication, at all levels
