An exciting opportunity has arisen for a suitably qualified, skilled and experienced individual to take on a leadership role in our team.

We are looking to recruit a CHC Nurse Assessor / Case Manager Learning Disabilities. This is a patient facing role and the post holder will deliver the end to end CHC / CCC / Individual funding process, supporting the CHC / Complex Case Team Leaders and the Operational Clinical Lead in all elements of service delivery.

Main duties of the job

Facilitating Continuing Healthcare/Funded Nursing Care assessments and reviews for patients across all aspects of care this includes appeals and retrospective assessments. This necessitates a high level of assessment skills and requiring tact, diplomacy and the ability to liaise effectively with colleagues, health care professionals, patients' carers and family members. The post holder will require Registered Nurse qualification.

Job description

Job responsibilities

  • The timely review of all Continuing Healthcare/Funded Nursing Care patients functioning as a central point for referral, assessment and support. This will contribute to the maintenance of the appropriate level of funding support, the accuracy of the Continuing Healthcare client database and the commissioning of appropriate continuing healthcare funded packages of care to meet the identified care needs of individual patients deemed eligible for support.
  • Offering continuing support and advice to family and carers in complex situations, necessitating a high level of interpersonal and communication skills; supporting the local resolution process in the case of complaints/appeals/retrospective cases and providing information on a timely basis.
  • Providing Specialist Nursing Advice to secondary care, primary care, nursing homes etc to facilitate the continuing healthcare process. Developing and maintaining effective and appropriate communication networks with health and social care professionals and also act as a resource to them.
  • Supporting community Matrons/District Nurses in their case management roles for complex cases who are eligible for continuing healthcare.
  • Ensuring there is an effective, up to date patient caseload plan which meets the requirements of the individual CHC service.
  • Co-ordination of the assessment process and where necessary writing key elements of the patient care plan to reflect presenting health needs.
  • Working with relevant health professionals, CHC colleagues, business support staff, senior leads and other support staff within the CHC team to ensure delivery of key targets and products required.
  • Supporting the publication of regular reports as may be required on progress within a given caseload of patients/sphere of service.
  • Exercising professional judgement as necessary when communicating information on behalf of the CHC service and application of the associated criteria.
  • Interpreting national and regional guidance relating to the implementation of the National Framework and subsequent developments within the CHC service.
  • Delivering formal and non formal education programs to Community and hospital staff including nursing/residential homes, GPs, community nursing and therapy staff relating to the assessment and management of CHC patient groups in line with the latest research, evidence and DOH/NICE guidance.
  • Ensuring timely "End of life" Care and reviews, following the national guidance for clients Fast Tracked through the CHC system.
  • Facilitating smooth patient flow across all aspects of the patients journey including liaison with primary care, secondary care, community venues and specialist services
  • Support and inform the targeting of resources, monitoring, implementation and evaluation of the tasks/projects by providing high quality support including complex information and analysis, communications and stakeholder management
  • Ensure accurate and open communication and co-ordination with a range of organisations and individuals, researching and drafting correspondence and papers and ensuring the management of specific tasks, lead reporting and analysis across a range of specialties, functions and projects
  • Be a key member of the team as well as supporting effective communication and stakeholder management, both internally and externally

Person Specification

Experience

Essential

  • Demonstrate relevant post registration experience as a registered nurse with some knowledge of case management or transferrable skills.
  • Experience of working both in a team and on own initiative.
  • Experience of working with the multi-disciplinary team and with a wide range of health and social organisations.
  • Previous experience in similar role in public sector.

Desirable

  • Experience of conducting CCC assessments and case management experience of children with complex health and behavioural needs.
  • Experience of supporting change management processes, along with evaluating these processes and an understanding of a performance lead service.
  • Further training or experience in project management, financial management or supporting change management processes.

Qualifications

Essential

  • Registered Nurse (Children, Mental Health, Learning Disability)
  • Educated to degree level in relevant subject or an equivalent level of experience working at a similar level.

Desirable

  • Mentorship qualification

Knowledge and Understanding

Essential

  • Must demonstrate an excellent knowledge and understanding of the care needs presented by individuals with a variety of conditions and disabilities.
  • Must have a working knowledge and experience of current government legislation around safeguarding strategy and the process of implementing this.

Desirable

  • Knowledge and awareness of resource management, including awareness of Personal Health Budgets (PHBs).
