Job summary
We are seeking to appoint a dynamic and enthusiastic candidate to the role of Team Leader for our Paediatric Hospital@Home service to provide a high level service to all patients and staff within the department. This is a job share with an existing Team Leader and is a fixed term contract to cover maternity leave. It is an excellent opportunity to join a motived dynamic team and newly established service.
Main duties of the job
An exiting opportunity has arisen within a BCYP H@H service. This is a new service that sits alongside an existing well established dynamic CCNT.
The post holder will provide maternity leave cover as the team lead for the BCYP Hospital at Home service. They will work within the multidisciplinary team to provide safe and appropriate management of children and young people with an acute illness which can be treated in the community. They will be working closely with the acute teams to ensure that patient safety is paramount.
They will be able to identify patients suitable for H@H and ensure a rapid discharge from ED or the ward. To plan, implement and evaluate their care. To review any re-admissions and identify potential reasons why this has occurred.

Education & Qualifications
Essential
- oRSCN or RN (Child)
- oRelevant First level degree or masters in appropriate subject or working towards
- oRecognised teaching/ mentorship qualification
Desirable
- oPrevious audit experience
- oworking towards CCN SPQ
- oNon-medical prescriber
- oCompleted an ANP course
