We
are seeking a skilled and compassionate Specialist COPD & Asthma Nurse to
join our innovative PCN healthcare team.
As
our specialist COPD & Asthma Nurse, you will play a crucial role in
providing specialised care and support to patients with COPD & Asthma. The
ideal candidate will have a strong background in COPD & Asthma management,
excellent communication skills, and a passion for delivering high-quality
patient care.
The
focus of this role will be to specifically focus on improving the quality of
care to patients with COPD & Asthma across the four MWEB PCN practices that
cover a combined patient list of 37,000 patients.
This
specialist role will complement the existing teams of clinicians in each
practice looking after this specific cohort of patients and will be a varied
role across the four practices and may involve working within a
Multi-Disciplinary Team for complex patients.

Specific focus will be on diagnosing,
initiation and management of medication in patients with COPD and Asthma
Provide Spirometry to enable diagnosis
and management of patients where appropriate
Use Feno monitoring where appropriate
Review patients with COPD & Asthma
holistically in order to identify other factors in their lives that affect
their quality of life that will have an impact on their COPD & Asthma

Reviewing patients
with COPD & Asthma and diagnosing, initiating and managing their
medications to improve their quality of life and try to achieve their agreed targets.
Work with the
existing practice teams to also ensure their patients with COPD & Asthma
have their other risk factors managed.
There may be times when more complex patients need to be discussed
within a Muti-Disciplinary Team setting and when this occasionally needs to
occur, you may need to:
a) Consult with patients,
their family, and the multi-professional team to undertake assessments of
patient need and devise and evaluate complex care plans;
b) Evaluate and analyse
clinical problems using their clinical knowledge, seeking out and applying
relevant evidence, enhanced techniques, interventions, and equipment to make
clinical decisions;
c) Delivers enhanced
clinical care in the context of continual change, challenging environments,
different models of care delivery, innovation and rapidly evolving technologies
using analysis and their underpinning knowledge to manage complex
interventions;
d) Teach and advise
patients and their families on how to manage their condition
e) Participate in clinical
audits and research projects where required and implements changes as required,
including the development, and updating of practice protocols / guidelines and
procedures locally;
f) Work within NICE
guidelines and Northants ICB formulary
g) Recognise boundaries of their practice and
know when and to whom patients should be referred;
h) Demonstrate initiative
and are creative in finding solutions to problems;
i) Hold joint responsibility
for team performance and service delivery.
As a
specialist nurse there may be a need for key wider responsibilities within the
PCN, for example to:
a) Facilitate
and support the development and delivery of innovative training and education
to staff, to improve competence and confidence, in line with best practice and
guidance related to area of expertise.
b) Participate
in and/or lead formal training events promoting best practice in area of
expertise.
c) Provide
professional and clinical leadership and mentorship, acting as a clinical role
model on the delivery of evidence-based practice.
d) Advise
and support on the development and delivery of work streams relating to Asthma/COPD
as part of the PCNs strategy.
e) To
improve identification, assessment, ongoing support for patients and their
family carers.

- Able to work in different environments, with different Primary Health Care Teams
- Experienced in the management of Asthma/COPD patients and the initiation of treatment in newly diagnosed Asthma/COPD patients.
- Degree in nursing
- Registered with the Nursing & Midwifery Council;
- Working towards level 7 qualification, have a Masters degree relevant to Asthma/COPD, or have 3+ years experience working as a Asthma/COPD nurse
- Be on the Association of Respiratory Technology & Physiology (ARTP) national register
Qualifications
