Job summary

A great opportunity to join an established Older Adult Mental Health Team who deliver services to the Worcester and Droitwich localities. As a service we understand the importance of work life balance and strive to find individual ways to ensure that our team members can achieve this, whilst ensuring we continue to deliver the best service to our patients.

The core working hours for the team are Monday to Friday 9-5. There is the opportunity for flexible working within this role around the core hours. This role easily allows you to blend working from home and working in the office.

We are a supportive team who regularly welcome students. The service has a strong focus on person centred care and understanding the expression of unmet needs in the person with Dementia. There are opportunities to develop specialist skills and you will receive regular supervision.

Main duties of the job

This post is a band 6 CMHN. You will have a caseload of patients who have been referred to the service for a diagnosis of Dementia or support with a Mental Health condition. Also, you will see patients who already have a diagnosis of Dementia and have a more complex presentation requiring specialist advice and treatment. You will manage the patients care from assessment to discharge.

It will be beneficial to have good time management skills as you will manage your own diary. You will attend MDT meetings weekly to discuss allocations and complex cases

You will develop the skills to undertake holistic assessment of patients and develop individual care plans and risk assessments with the patient.

Job description

Job responsibilities

Act as a role model to colleagues, always seeking to maintain the highest standards of professionalism

Understand and apply person centred care at every stage of contact with the patient.

Work as part of the Multidisciplinary team across a range of clinical pathways

Manage a caseload of patients with complex Dementia needs, using evidence based / patient centred principles to assess, plan, implement and evaluate interventions.

Maintain robust communication with patients, carers and other local agencies. Maintain accurate timely clinical records.

Support patients and carers to achieve agreed outcomes and goals utilising the recovery model.

Provide leadership for junior staff, through supervision and appraisal.

Person Specification

Experience

Essential

  • Documented evidence of continuing professional development
  • Significant post registration experience including experience within mental health
  • Minimum 1 year within / related specialty
  • Extensive clinical experience including individual and group work

Desirable

  • Experience in a relevant clinical setting
