Job summary
We are looking to recruit a
motivated, committed and enthusiastic Practice Nurse to join our Practice team.
The successful candidate will
need to be highly motivated with excellent communication and interpersonal
skills. They will be responsible for ensuring the delivery of safe and
effective nursing care to the whole practice population, working closely with the
GPs, ANP's, PAs, Paramedics, Pharmacists, Practice Manager and nursing team to deliver the practice working priorities.

Main duties of the job
Clinically, the focus of the role
is management of patients with long-term conditions and general nursing
interventions for all patients.
It is essential you have
knowledge of COPD & Asthma management, Diabetes management, Hypertension, family
planning /contraception, cervical cytology, childhood immunisations and vaccines.

Job responsibilities
To be responsible for the delivery of practice nursing services, working as part of the practice team, delivering care within their scope of practice.
The practice nurse will be
responsible for a number of clinical areas such as health promotion, chronic
disease management, health prevention, immunisations and travel vaccines, infection control, safeguarding, well woman and well man management, as well
as supporting the management team in the reviewing of clinical policies and
procedures.
