work to develop and deliver the medicines management and medicines safety
agenda within Primary Care 24 (PC24).

The post-holder will work with
clinical colleagues to ensure the highest standards of care are provided for
all registered and temporary patients and will work across the organisation to
become a core member of the team of healthcare professionals.

Job description
Job responsibilities
The post-holder will be expected to undertake
all the normal duties and responsibilities associated with a Clinical
Pharmacist working in General Practice and the out of hours setting including:
Develop, produce, and implement
medicines policies and procedures.
Develop, produce, and implement
medicines management strategies to ensure compliance with CQC, QoF, legislation
and local quality frameworks.
Develop, produce, and implement
clinical audit in relation to medicines.
Incorporate the objectives of
medicines optimisation into every day general practice, including the aims of
national and local strategies.
Lead, plan, develop and support the
introduction of new working processes within the services.
Undertake clinical structured
medication reviews and repeat prescribing when necessary.
Reconcile medicines following
discharge from hospitals and intermediate care, including identifying and
rectifying unexplained changes, and working with patients and community
pharmacists to ensure patients receive the medicines they need.
Set up and manage systems to ensure
continuity of medicines supply to high-risk groups of patients (e.g., those
with medicine compliance aids or those in care homes).
Identify cohorts of patients at high
risk of harm from medicines through practice computer searches, including risks
that are patient related, medicine related, or both.
About you
- Minimum of 5
years post-qualification experience
- An appreciation of the nature of primary care
prescribing, concepts of rational prescribing and strategies for improving
prescribing
- In depth
therapeutic and clinical knowledge and understanding of the principles of evidence-based
healthcare
- Experience of
medication review, face to face and remotely
- Experience of
repeat prescribing and prescribing for long term conditions within competency
- Experience and
knowledge of developing and undertaking clinical audit
