Job summary
We are looking for an enthusiastic and motivated doctor to join our team in providing front door frailty services for the population of the London Borough of Newham.
You will be based in the emergency department, providing comprehensive geriatric assessment to frail older patients as part of a multi-disciplinary team including emergency department practitioners, therapists and nurses.
This role provides good experience in the management of elderly and frail patients with the opportunity to develop skills in comprehensive geriatric assessment, deprescribing and communication. There is the opportunity to work with more autonomy and to develop discharge planning knowledge, as well as experience in receiving and vetting referrals. The work is rewarding and has an impact on improving patient care and avoiding admissions.
The role provides a variation from ward work and offers clinic time, audit and QIP opportunities. It would suit a candidate with geriatric medicine experience looking to develop their skills further or those considering geriatric medicine as a long term career option.
The working hours are Mon- Fri 9-5 and the post does not include a fixed on call commitment.
Main duties of the job
Inpatient COE services:
Thistle AAU - Acute COE with predicted LOS < 24 hrs (13 beds).
Thistle Short Stay - Acute COE with predicted LOS <72 hrs (13 beds).
Silvertown Ward - Acute COE with predicted LOS >72hrs (24 beds).
Custom House Ward - Complex COE (20 beds)
Heather Ward - Acute Stroke Unit (13 beds). Highly performing ASU works alongside RLH HASU regional thrombectomy centre.
Each consultant is responsible on average for 13-16 inpatients, which is variable depending on season.
Outpatient COE services:
We provide a wide range of outpatient clinics. These include general COE, diagnostic memory clinic, falls clinic, stroke and TIA and complex CGA. There are daily HOT clinic slots incorporated into all clinics.
Expected new to follow up ratio in clinic of 3 new to 5-6 follow up patients.
Other COE services provided:
Frailty - We have embedded frailty screening within the Emergency Department for early recognition of frailty. Our front door frailty service has been piloted in the ED over the past six months with excellent results. This post will support us to continue to expand our frailty service.
