We are a friendly, resilient practice looking to welcome an enthusiastic and committed GP to our fantastic team with the possibility of working over 2 sites (one of which is a training practice) with a mixed and diverse patient population and fantastic support staff.

Main Duties
The post holder will work predominately at Harbour Medical Practice where there is a strong focus on multi-disciplinary teamwork. The post holder will receive leadership and support, listed below is a non-exhaustive list of main duties.
- Support the progression of patient pathways
- . Provide medical review to include assessment and management of thedeteriorating patient.
- Participate in regular MDT board meetings to review patient care
- To support the screening / review of referred patients from varying clinicalservices5. The post holder will be required to maintain their knowledge and skills withutilisation of relevant e-portfolio. Study leave provision will be available.6. To participate in training of advanced clinical practitioners and medicalundergraduates if applicable.7. To contribute towards mortality review processes, clinical audit and clinical governance & educational meetings.8. The post holder will share with colleagues and other intermediate level staffin dictation of documentation in respect of patients, where appropriate, inaddition to general administrative duties as part of their role.

SPECIFIC DUTIES/RESPONSIBILITIES
Clinical Responsibilities:-
- In accordance with the Practice timetable, as agreed, the post-holder will make him/her-self
available to undertake a variety of duties including surgery consultations, telephone consultations
and queries, visiting patients at home, checking and signing repeat prescriptions and dealing with
queries, paperwork, reports and correspondence in a timely fashion.
Making professional, autonomous decisions in relation to presenting problems, whether selfreferred or referred from other health care workers within the organisation.
Assessing the health care needs of patients with undifferentiated and undiagnosed problems.
- Screening patients for disease risk factors and early signs of illness.
- In consultation with patients and in line with current Practice disease management protocols,
developing care plans for health.
Providing counselling and health education.
Recording clear and contemporaneous consultation notes to agreed standards.
- Collecting data for audit purposes and complying with QOF requirements.
Delivering enhanced services.
Compiling and issuing computer-generated acute and repeat prescriptions (avoiding hand-written
prescriptions whenever possible).
Prescribing in accordance with the Practice prescribing formulary (or generically) whenever this is
clinically appropriate

Qualifications
Essential
- Essential
- Medical degree. Membership of the Royal College of General Practitioners or
- equivalent in training if approved by the Royal College of General
- Practitioners. Certificate of MRCGP.
- Registered GP with the GMC (included on the GP Register).
- Included on GP Performers List.
- Possesses the right to work in the UK.
- Certificate of satisfactory immune status re: Hepatitis B and other relevant viruses.
- Current General Practice experience with up-to-date GMC appraisal/revalidation.
- Childhood Surveillance.
- At least 6 months each of acute A&E, medical and surgical experience. Confident
- in Womens Health.
- Demonstrable ability to relate well to challenging patients, carers, relatives and
- staff.
- Experience of working with the socially deprived, nursing home, chronic medical
- conditions, mental health and HIV positive patients.
- Full driving licence, with use of own car.

- Publications in the field of general practice, general medicine or surgery.
- Firsthand experience of developing services in a constantly changing environment.
- Ability to demonstrate an understanding of private general practice and how best to
- dovetail this with NHS services.
- Knowledge of adherence to the CQC Fundamental Standards.
- Understanding of contracting, finance, service delivery and activity monitoring.
