Job summary
The Integrated Care Partnership have an exciting
opportunity for an experienced practice nurse to join our friendly practice
team. You will be supported by a fantastic MDT team, comprising of GPs, HCAs,
Practice Paramedics, Physiotherapist, Mental Health Nurse and Clinical
Pharmacists, along with a very friendly patient services team and supportive practice
management team. We are happy to consider both full time and part time
applicants.
Main duties of the job
The post holder will be an experienced registered primary care nurse who, acting within their professional boundaries, will provide care for the patient from initial history taking to evaluation of care. They will demonstrate safe, clinical decision-making and will work collaboratively with the general practice team to meet the needs of patients, supporting the delivery of policy and procedures, and providing nurse leadership as required. You will need to be an autonomous, motivated and enthusiastic team player.
The successful candidate will be expected to perform and manage all Practice Nursing duties, including, but not exhaustive;
Primary Immunisations
Travel Immunisations (NHS)
Wound Care/Management
Cervical Screening
ECGs
NHS Health Checks
Stock Management
Vaccine Management
Chronic Disease Management (Asthma, COPD, Diabetes etc.) desirable

Job description
Job responsibilities
Job Responsibilities:
Provide assessment, screening and treatment services and health
education advice
Provide nursing treatments to patients in participation with general
practitioners or independently to agreed protocols.
Provide general and specific health screenings to the Practice patients
(within agreed protocols) with referral to general practitioners as necessary.
Advise patients on general health care and minor ailments with referral
to GPs as necessary.
Deliver vaccinations and immunisations as required.
Meet the needs of patients presenting for wound care
Ensure the maintenance of equipment and stock levels relating to patient
care.
Pathological specimens and investigatory procedures
Undertake the collection, storage and despatch of pathological specimens.
Perform any investigatory procedures requested by the GPs, including
blood tests and ECG readings
Support patients and colleagues in adopting sound infection prevention
control measures and adhere to infection prevention control local and national
guidelines
Identify, diagnose and manage patients at risk of developing a long-term
condition, as appropriate
Work with other health care professionals to diagnose, monitor, manage
& treat long-term conditions including non-drug-based treatment methods.
Support patients to adopt health promotion strategies that promote
healthy living, and apply principles of self-care
Demonstrate sensitive communication styles to ensure patients are fully
informed and consent to treatment.
Participate in planning and implementing changes within the area of care
and responsibility
Work collaboratively with colleagues, MDT and other healthcare
professionals.
Administrative and professional
responsibilities:
Participate in the administrative and professional responsibilities of
the Practice team.
Maintain accurate patient records of all consultations and treatments
and record in the patients notes and on the clinical computer system as
appropriate.
Ensure accurate completion of all necessary documentation associated
with patient health care and registration with the Practice.
Ensure collection and maintenance of statistical information required
for regular and ad hoc reports, audits and research programmes.
Attend and participate in Practice clinical meetings on a regular basis.
Assist with the Practice philosophy, strategy and policy and develop
appropriate protocols.
Professional/Personal development
Maintain continued education by attendance at courses and study days as
deemed useful or necessary for continuous professional development, and
participate in any training programme implemented by the Practice as part of
this employment.
Participation in an annual individual performance review, including
taking responsibility for maintaining a record of own personal and/or
professional development.
Taking responsibility for own development, learning and performance and
demonstrating skills and activities to others who are undertaking similar work
including participation in clinical supervision and acting as a positive role
model.
Support the development of others in order to maximise potential.

Qualifications
Essential
- Registered General Nurse qualification and up to date with NMC registration
- Practice nurse experience and experience working in primary care
