An exciting opportunity has arisen for aDistrict Nurse / Community NurseBand 6 tojoin our Surrey HeathCommunity Nursing Teamto help us continue to change things for the better.

Surrey Heath Community Nursing works across 4 teams and has a Single Point of Access office in Camberley where all referrals and phone calls are triaged, and is part of the Integrated care Team.

The community nursing team covers8am to 8pm, 365 days a year. Our shifts include a mixture of 8am to 4pm, 9am to 5pm and 12pmto 8pm. You would do one late shift a week, and join the rota to work 1 weekend in 4. We are also happy to consider part time hours.

Main duties of the job

Within this role you will:

  • Provide leadership and management to your team.
  • Have responsibility for your caseload and have skills and competencies to provide excellent patient care in their homes.
  • Undertake full nursing assessments of patients, including those with multiple co-morbidities and complex health and social needs.
  • Be responsible for agreeing the patients care plan, liaising with others and adapting the care and treatment delivered according to need and patient choice.
  • Work with the Integrated care system and MDT to provide holistic care for your patients.
  • Responsible for contributing to the holistic assessment, planning delivery and evaluation of care to patients within their own homes.
  • implement, monitor and maintain high standards of care delivery at all times.
  • Care for patientsand their families in the way thatyou believe is best
  • Deliver our high service standards, sharing bestpractice, and actively improving the way we work.

Job responsibilities

You will be aNMC/1stlevel registered Nurse with 2 years community experience.You will have the ability to work both independentlyand alsoas part of a wider multidisciplinary team. It is expected that youshould have strongcommunication and organisation skills and a flexible approach which will ensureyou deliver positive experiences to patients at every stage of theirhealthcare journey. You will have a full UK driving license with access to a car for work purposes.

You will also need to be confident using IT systems and be able to demonstrate working knowledge of Microsoft Office packagesand haveworking knowledge of an i pad which is used for mobile working.
