Relevant to both blocks: Working as part of the MDT with rest of medical team, on call consultant if relevant, nursing staff and allied healthcare professionals, including Psychological Services Team Participation in weekly hospice-wide MDT meeting Clinical supervisor will provide time for meetings at beginning, middle and end of placement, and monthly sessions between as needed Trainee to lead on their workplace assessment needs, but will be supported by hospice Time to get involved with clinical audit and other quality improvement projects will be supported and encouraged Weekly tutorial time with consultant offered, trainee-led, based on individual learning needs Participation in weekly medical team meeting, including presenting on relevant topics, and journal club Managing the rota for the weekly medical team meeting Supporting and teaching medical students SPA time will be allocated Opportunities for management experience will be encouraged, such as taking part in clinical governance meetings and taking a lead in certain areas

1-in-6 ON CALL DUTIES

Week nights:From 5pm 9am, be available for any medical needs the ward might have, as during the day. Providing you live within a reasonable distance, you may return home and be contactable from there. (If your driving time is over 45 minutes, please discuss with the consultant.) Duties include: Admitting any patients who arrive during that period (most admissions are planned for daytime but emergencies or late arrivals due to transport delays occasionally need clerking during the out of hours period) Giving phone advice to nursing staff Returning to the hospice to see patients who have deteriorated unexpectedly and need a review of their care plan Some medications can be prescribed remotely if the patient does not need review in person The nursing staff or on call manager may ask you to attend for other reasons, if they deem they need support in a matter It is wise to phone the nurses around 10pm or before you go to bed, so that any small niggles or questions can be resolved; this may avoid the nursing staff having to ring and wake you in the night There is a consultant on call available at all times in the out of hours period, on the phone

Weekend days: Attend the ward and do a ward round for any patients who need to be seen. These will either have been identified on the Friday by the ward medical team (or yourself), or be identified by either the nurses, or the patient/family may request to see you. Not all patients may need to be seen, e.g. if purely awaiting discharge Completion of the Medical Examiner Referral and Medical Certificate of Cause of Death for patients who have died over the weekend period. Please bear in mind the potential need to do this when conducting a ward round ensure you have at least met all patients for whom certification may be necessary. (Patients who have died over the weekend will have their death verified by the nursing staff.) Once all anticipated duties are completed, the rest of the on call may be done from home, returning that day only if needed

Qualifications

Essential

• MBBS or equivalent medical qualification

Desirable

• MRCP (UK)
• MD, PhD or higher degree

Career Progression

Essential

• Evidence that career progression is consistent with personal circumstances
• Evidence that present achievement and performance is commensurate with totality of period of training
• At least 24 months' experience in medicine or an IMT, ACCS or equivalent programme (not including Foundation modules) by the time of commencement of ST3 training

Eligibility

Essential

• Hold full registration and licence to practise with the GMC
• Evidence of achievement of Foundation competences from a UKFPO affiliated Foundation Programme or equivalent by time of appointment in line with GMC standards/ Good Medical Practice including: ogood clinical care omaintaining good medical practice ogood relationships and communication with patients ogood working relationships with colleagues ogood teaching and training oprofessional behaviour and probity odelivery of good acute clinical care

Desirable

• Evidence of completion of CT/ST1 or equivalent competences in medicine at time of application, and CT/ST2 competences in medicine by time of appointment, supported by evidence from work-based assessments of clinical performance and Multisource Feedback (DOPS, Mini-CEX, CBD, ACAT) or equivalent.
• Previous clinical experience in Palliative Medicine

Language Skills

Essential

• All applicants to have demonstrable skills in written and spoken English that are adequate to enable effective communication about medical topics with patients and colleagues
• That applicants have undertaken undergraduate medical training in English; or have the following: -PLAB 1 -PLAB 2

Clinical Skills

Essential

• Valid BLS (Resus Council UK)
• Competence in the immediate management of medical emergencies
• Appropriate knowledge base, and ability to apply sound clinical judgement to problems
• Proficiency in a range of medical procedures
• Able to work without direct supervision where appropriate
• Able to prioritise clinical need
• Able to maximise safety and minimise risk

Desirable

• Valid ALS (Resus Council UK)
• Personal attributes: shows aptitude for practical skills, e.g. manual dexterity.

Academic / Research skills

Essential

• Demonstrates understanding of research, including awareness of ethical issues
• Demonstrates understanding of the basic principles of audit, clinical risk management, evidence-based practice, patient safety, and clinical quality improvement initiatives
• Demonstrates knowledge of evidence-informed practice

Desirable

• Evidence of relevant academic and research achievements, e.g. degrees, prizes, awards, distinctions, publications, presentations, other achievements.
• Evidence of active participation in audit
• Evidence of involvement in teaching students, postgraduates and other professionals
• Evidence of participation in a teaching course
