The post-holder will:

Assist in the delivery and provision of General Medical Services, Enhanced Services, and work with the GP Collaborative, in accordance with the GMS and other practice contracts. Continuing professional development opportunities, and regular practice meetings.

Make him/her-self available to undertake a variety of duties including surgery consultations, telephone consultations, visiting patients at home and in nursing homes, checking and signing repeat prescriptions and dealing with queries, paperwork and correspondence in a timely fashion.

Be required to be on-call for clinically urgent and emergency problems and consequently be included in the doctors on-call- rota.

Make professional, autonomous decisions in relation to presenting problems, whether self-referred or referred from other health care workers within the organisation

Cover all the clinical management of the patients in surgery as appropriate including initiating investigations, reviewing results, making referrals to secondary care or to other providers as appropriate

Develop care and treatment plans in consultation with patients and in line with current Practice disease management protocols as well as wider guidance such as that from NICE

Play a role in the practice achieving QIF/ Collaborative priorities and other key performance indicator targets.

Main duties of the job

The role of salaried GP within the practice will focus on a mix of routine sessions and on call sessions. The on call sessions are Rota'd amongst all GP's equally on a pro rata basis to ensure fairness.

The practice offers a mixed approach to appointments, including face to face appointments and telephone consultations, both available on the day for acute presentations and pre-bookable appointments for more routine matters. Daily patient contacts amount to 30.

The practice uses Engage Consult as its workflow mechanism with a dedicated care navigation team working alongside the on call GP to ensure appropriate allocation of work throughout the day to the clinicians.

We are currently a Vision practice but will shortly be transitioning to EMIS Web, bringing with it EPS capability.

We are a forward-thinking, progressive surgery engaging in new processes and focussing on efficiency gains with clinician wellbeing at the forefront of our processes, ensuring the working environment is one that is sustainable for all working within the surgery.

As a training practice we also currently have 2 registrars at the practice, and there is opportunity for all GP's to contribute to their development through hot review management and the opportunity for all to perform tutorial workshops with the trainees, with protected time assured for these sessions.

Special interest services are also welcomed and valued at the practice as agreed in line with business needs.

Pontardawe Primary Care Centre is a friendly and forward thinking practice within the Upper Valleys cluster of Swansea Bay University Health Board. We are looking to recruit into our close-knit team. The practice consists of 4 GP partners, 3 salaried GPs, Practice Business Manager, an Advanced Nurse Practitioner, 3 practice nurses, a Pharmacist-led pharmacy team, health care support workers and a dedicated team of administrative staff. Clinical Responsibilities:

The post-holder will:

Screen patients for disease risk factors and early signs of illness

Cover all the clinical management of the patients in surgery as appropriate including initiating investigations, reviewing results, making referrals to secondary care or to other providers as appropriate

Develop care and treatment plans in consultation with patients and in line with current Practice disease management protocols as well as wider guidance such as that from NICE

Ensure appropriate follow up of patients

Record clear and contemporaneous computerised consultation notes to agreed standards

Collect data for audit purposes

Compile and issue computer-generated acute and repeat prescriptions, avoiding hand-written prescriptions whenever possible

Prescribing generically whenever appropriate and in accordance with the Practice prescribing formulary, NICE guidance, Good Prescribing Practice

Work with and support other members of the primary health care team in delivering high quality primary care

Promote the application of standards and codes of Practice set by the Royal College of General Practice, General Medical Council and Department of Health

Play a role in the practice achieving QIF/ Collaborative priorities and other key performance indicator targets.

Other Responsibilities within the Organisation:

Awareness of and compliance with all relevant Practice policies/guidelines, e.g. prescribing, confidentiality, data protection, health and safety

Commitment and contribution to achievement of the highest possible quality standards such as the QOF/ Cluster priorities and other agreed Key Performance Indicator targets

Attendance at regular in-house clinical, educational meetings and training as well as events with other agencies as appropriate

Using appropriate infection control procedures, maintaining work areas in a tidy and safe way and free from hazards
