Job summary

Kingsway Medical Practice in Rochdale is looking for a Prescribing Practice Nurse to join our friendly team as we look to better serve the needs of our patients. You will be supporting patients to be healthy by monitoring long term conditions, and by focusing on health prevention and screening. You will be working with a team of GPs, a Nursing team and an excellent admin/reception team that keep the show on the road. There will be professional development opportunities, including cross-practice meetings with other Practice Nurses and opportunities for further training.

Main duties of the job

The post holder will be responsible for providing a comprehensive, autonomous, efficient and professional clinical nursing service to the practice patients, including referral to other Health Care Professionals, in accordance with practice approved guidelines. To encourage the development of management and prescribing protocols. To participate in nursing needs assessment and discharge nursing responsibilities to patients when necessary.

Job responsibilities

Screening

Undertake opportunistic screening BP checks, height, weight, BMI, urinalysis, smoking status etc. Perform new patient registration checks

Undertake opportunistic venepuncture

Health Promotion

Recognise patients readiness to change in supporting their attempts to change unhealthy behaviour Provide healthy eating advice to individuals or groups including the underweight

Provide smoking cessation advice individually in opportunistic consultations and provide nicotine replacement therapy / champix under agreed protocol

Provide individual advice to patients about increasing physical activity or exercise

Vaccination and Immunisation

Administer injections under an individual prescription or Patient Group Directives including travel vaccinations, childhood immunisations, routine adult immunisations, B12 injections and Zoladex etc

Ensure safe storage, rotation and disposal of vaccines and drugs

Regular stock taking and ordering of vaccinations

Infection Control

Support patients and colleagues on adopting sound infection control measures

Apply infection control measures within the practice according to local and national guidelines,

Cardiovascular disease

Contribute to the implementation of the National Service Framework

Contribute to the maintenance of the practice CVD register

Advise patients on lifestyle management and drug compliance for reducing the risk factors of Hypertension, Angina, Myocardial infarction, Cerebrovascular accident

Where appropriate and where individual clinical management plans have been agreed with the GP, the Practice Nurse will titrate medication, in accordance with practice protocols, for patients with hypertension and raised cholesterol.

Child Health/Child Protection

Recognise the signs, symptoms and categories of child abuse

Understand the importance of recognition, observation, documentation and communication

Is aware of child health procedures, statutory local procedures and points of referral

Has knowledge of other practitioners and agencies roles

Understands basic legal issues around child abuse

Quality Outcomes Framework (QOF)

Contribute towards QOF by using the clinical computer system effectively and inputting patient information during consultations

Assist the practice in achieving QOF targets by acting on major alerts/messages on patients records and by opportunistic collection of data

Diabetes

Support patients in the management of non-insulin dependant diabetes

Undertake opportunistic screening for the diagnosis of diabetes

Support and provide basic education for diabetic patients

Contribute to the maintenance

Learning and Development

Make effective use of learning opportunities within and outside the workplace, evaluating their effectiveness and feeding back relevant information

Provide an educational role to patients, carers, families and colleagues in an environment that facilitates learning.

Clinical Duties as an Independent Prescriber

To work as an independent non-medical prescriber within the area of COPD / Asthma taking responsibility for any clinical management required as well as responsibility for prescribing where necessary and the appropriateness of any prescription issued.

Independent prescribers will be responsible for prescribing a range of drugs within their competence in accordance with Practice/CCG guidelines and the BNF.

To write prescriptions in accordance with the Practice/CCG policy taking full responsibility for the medicines prescribed.

Ensure the appropriate records are made in the patients records which identify you as the independent prescriber.

To ensure that contemporaneous records which are unambiguous and legible are maintained and kept in accordance with Practice policy and professional codes of conduct.

To undertake auditing and monitoring of prescribing in your own designated area of practice.

To ensure the safe handling of FP10 prescription forms and to report any loss or theft accordingly.

In the event of adverse drug reactions, to be accountable for ensuring that the GP is advised accordingly and that the incident reporting process is followed

To be accountable for ensuring that training, CPD and clinical supervision sustains your independent prescribing status.

To have an in depth understanding of the risks associated with NMP and the management of side effects and errors.

Person Specification

Qualifications

Essential

  • Registered First Level Nurse
  • Successful completion of a non-medical prescribing course

Desirable

  • Relevant Nursing degree
  • Courses in Chronic Disease Management

Knowledge

Essential

  • Knowledge of needs of patients with long-term conditions. Aware of accountability of own role and other roles. Knowledge of health promotion strategies. Awareness of clinical governance issues in primary care. Knowledge of patient group directions and associated policy. Awareness of the concept of whole person healthcare.

Desirable

  • Ability to identify determinants on health in the local area. Knowledge of public health issues in the local area. Awareness of local and national health policy. Awareness of issues within the wider health economy.

Experience

Essential

  • Minimum 2 years post registration experience.
  • Management of long-term conditions.
  • Involvement in implementing and using protocols and clinical guidelines. Experience of audit.
  • Experience as an independent non-medical prescriber.

Desirable

  • Experience within General Practice.
  • Nursing experience of running Chronic Disease management clinics e.g., Asthma, Diabetes etc.
  • Experience as an independent non-medical prescriber.

Skills

Essential

  • Clinical skills; immunisation and vaccination, phlebotomy, examination skills e.g., BP, pulse, BLS.
  • Change-management skills and ability to support patients to change their lifestyle. Communication skills, both written and verbal.
  • Ability to communicate difficult messages to patients and families.
  • Negotiation and conflict management skills. IT skills with clinical systems such as EMIS Web.

Desirable

  • Clinical skills; cervical cytology, diabetic foot checks, AED.
  • Experience of teaching and mentorship in a clinical setting.
