Job summary

Bury Hospice delivers high-quality, compassionate palliative and end of life care to local people living with life-limiting illness. An exciting opportunity has arisen for a highly motivated individual to join our Community Outreach Team supporting patients and families in their own homes. We offer proactive holistic support throughout the patients journey.

Main duties of the job

The Team works in collaboration with District Nurses, Clinical Nurse Specialists and other members of the multidisciplinary team to provide care and support to patients and their families/carers in their own home.

The usual hours of work for this role are 22.5 hours per week, 7.5 hours shifts, worked over 3 days between Monday and Friday. However, the role will include periodic rotation onto the Domiciliary Hospice at Night Respite care service to cover annual leave of the regular night team. Shifts will be worked 9pm-7am over 2/3 nights, Tuesdays to Thursdays. Enhancements will be paid for the unsocial hours.

MAIN DUTIES & RESPONSIBILITIES

  • Offer face to face holistic support to patients and families in their own home and the hospice environment for patients attending as outpatients.
  • In line with the lone worker policy offer situational respite care and practical support on an individualised patient basis.
  • To signpost patients and families/carers with their agreement to other health care professionals/services.
  • Deliver personal care when required as part of an agreed individualised plan of care.
  • Establish and maintain effective communication with patients, relatives and carers and members of the multidisciplinary team regarding aspects of care.
  • To work alone in a patients home without direct supervision.
  • To recognise changes in patients needs and phase of illness and report these to the Nurse in Charge.
  • To ensure that all care/support given is reported and documented, at the earliest opportunity, within the agreed systems, and share it with the relevant people in the team in a timely manner.
  • To obtain consent from patients/carers before commencing any care related task.
  • To contribute to service development.
  • To attend and participate in Team Meetings.

Experience

Essential

  • Healthcare experience in a Hospice, Hospital or Community setting.

Desirable

  • Previous experience of lone working.

  • Good organisational and interpersonal skills.
  • Ability to work independently and in a team.
  • Able to document concise, legible and accurate nursing notes.

  • Basic IT skills and able to access the internet and use e-mail.
  • Manual dexterity.

  • Full clean and current driving licence required with access to own vehicle for business purposes.

Qualifications

Essential

  • GCSE Qualification or equivalent.
  • NVQ Level 2 in Care and/or Care Certificate.

Desirable

  • NVQ Level 3.
