Job summary

We are currently seeking registered nursing associates who are looking for an opportunity to develop skills in rapid response, discharge to assess and community rehabilitation management of acutely unwell clients in the community. You will be working under the guidance of a registered nurse and/ or multi-disciplinary clinicians who will delegate, to support assessment and treatment of patients to achieve medical stability and maintain functional ability, aiming to prevent unnecessary hospital admissions. The focus is on delivering high quality, person centered. compassionate care .

  • They will be part of the multi-disciplinary/integrated team delivering planned care to patients working within the standards of proficiency outlined by the Nursing and Midwifery Council and in accordance with HRCH policies and protocols, with access to advice and supervision as required.
  • To work with other healthcare professionals and agencies in the delivery of services to prevent hospital admission, support timely and safe discharge and establish effective personalised care and treatment plans to support people to remain at home.
  • To provide education and support to individuals and their families to promote self-care strategies and independence.

Main duties of the job

  • To work independently within the HRCH scope of practice of a nursing associate, under the leadership of registered nurses.
  • Demonstrate understanding of all elements of the nursing process and be able to assist the registered nurses in the on-going assessment, planning, management, and evaluation of care.
  • Follow plans of care prescribed by the registered nurse with individual patients and carers in their own homes. Provide high standards of care encompassing a rehabilitative approach where appropriate. Actively report any changes in a patient's condition in a timely way to the registered nurse.
  • Undertake clinical tasks including but not limited to urinalysis, blood glucose monitoring, wound care including compression bandaging, venepuncture, medicines administration, insulin injections, continence care, stoma care, catheterisation and condition monitoring.
  • Perform and record clinical observations including blood pressure, temperature, respirations, pulse and calculate a NEWS score
  • Administer medications in accordance with NMC code.

Qualifications

Essential

  • Foundation Degree in Nursing. Trained Nursing Associate Registered with the NMC.
  • Evidence of ongoing professional development

Desirable

  • Evidence of European Computer Driving licence or equivalent

Knowledge

Essential

  • Nursing Associate understanding of requirements of NMC professional revalidation
  • Knowledge of when to seek advice and refer to a Registered Nurse or other healthcare professional
  • Knowledge and understanding of appropriate procedures and treatment plans, and willing to develop new skills and competence
  • Safeguarding Adults & Children

Desirable

  • Understanding of the impact of Mental Health e.g., dementia, depression, on the management of medical and long term conditions

Experience

Essential

  • Understands the scope of the role of the Nursing Associate in context of the nursing and interdisciplinary team and the organisation, and how the role may contribute to service development
  • Experience of working in a health and social care Setting
  • Previous experience of clinical supervision and reflective practice

  • Previous experience of working in community-based team
