We have a great opportunity for an enthusiastic Practice Nurse to join our large, vibrant, and friendly teaching Practice. We are looking for the right person to join our Nursing team that includes a Lead Nurse, Practice Nurses and Healthcare Assistants.

We pride ourselves on delivering high quality holistic care to our patient population and have a culture in learning and teaching in all we do. We are looking for a forward thinking experienced Practice Nurse who has the qualifications and/or experience to support our patients with long term health conditions, and be comfortable with the ever changing day to day world of General Practice.

Main duties of the job

The successful applicant will have the experience and qualifications to advise patients in health prevention, carrying out screening and early management of long term health conditions.

The successful candidate must:

  • Be a certified Registered Nurse
  • Have a current registration with the NMC
  • Be registered with the relevant professional body
  • Have experience in a range of nursing clinical skills including cervical cytology (preferable), immunisation and vaccinationsand long term conditions such as respiratory conditions & diabetes

Job responsibilities

Develop, implement and embed health promotion and well-being programmes

Manage patients presenting with a range of acute and chronic medical conditions, providing subject matter expert advice to patients

Implement and evaluate individual specialised treatment plans for chronic disease patients

Identify, manage and support patients as risk of developing long-term conditions, preventing adverse effects on the patients health

Provide wound care (ulcer /Doppler etc.) to patients

Provide specialist clinics such as contraception

Undertake the collection of pathological specimens

Provide travel medicine services

Request pathology services as necessary

Process and interpret pathology and other test results as required

Provide chronic disease clinics, delivering patient care as necessary, referring patients to secondary / specialist care as required

Qualifications

Essential

  • Registration as a Nurse
  • Membership with the relevant professional body

Desirable

  • Qualified Nurse Prescriber
  • Minor Illness Qualification
