Job summary

You will assist in the management and lead a team of nurses, ensuring that they carry out their duties consistent with Stockport Localities policies, procedures and thresholds. The role of the team is to ensure that the Stockport population is able to access eligible care in a safe and timely manner. This will involve managing patients, families and other stakeholder's expectations. You will be responsible for assuring that patients are receiving safe and effective care from a large range of providers under a variety of contracts and agreements.

You will support and manage the Nurse Assessorteam working with both adults and children who are eligible for NHS provision of Continuing Healthcare and NHS Funded Nursing care. This role will be responsible for ensuring Stockport Locality provides equitable and safe, robust packages of care. To support this management role, the person must hold an appropriate nursing or professional qualification. They will need to have experience of managing a team, being able to actively work independently and to make judgements and recommendations to the executive team in line with operational policies.

Main duties of the job

KEY DUTIES AND RESPONSIBILITIES

  • The post holder will be required to manage relationships with the Clinical Nurse Assessors, maintain networks internally and externally.
  • The post holder will be required to communicate both highly complex and sensitive information internally and externally.
  • Actas a role model to the team of Continuing and Complex healthcare Nurses.
  • Maintains relationships with providers, other commissioners and other agencies often when communicating sensitive issues or in pressured environments. Uses these relationships to ensure the provision of high-quality care for Stockport patients.
  • Has the ability to persuade board members and senior managers of the importance of the programme and to negotiate and motivate stakeholders.
  • Participates in relevant internal and external working groups, services and initiatives to provide information and expertise to a range of providers.

COMMUNICATION

  • Developed communication skills for delivering key messages to a range of stakeholders both internal and external (including outside the NHS)

INFORMATION RESOURCES, ANALYSIS AND DECISION MAKING

  • Develop the acquisition, organisation, provision, and use of knowledge and information.
  • Evaluate existing recording systems and seek modernisation and continuous improvement.
  • Drafting reports summarising status on issues, appraising outcomes, and providing progress reports for the Head of Department.
  • Provides specialist advice, support and guidance to staff, managers and clinical leads from a wide range of organisations on the subject of Continuing and Complex Health care.
  • Responsible for the day-to-day staff management, which will include responsibility for supporting appraisals, development of staff, recruitment and where necessary processes such as grievance and disciplinary matters.

PLANNING & ORGANISATION

  • Ensure projects and initiatives are delivered on time to quality standards and in a cost effective manner, adjusting plans as required.
  • Determine short, medium and long term business plans, achieving quality outcomes.
  • Develop and support a system wide Continuing and complex healthcare training and awareness programme.
  • Evidence of planning and delivering work and projects and services on time.
  • Demonstrated capability to plan over short, medium and long-term timeframes and adjust plans and resource requirements accordingly
  • Experience of setting up and implementing internal processes and procedures
  • Experience of identifying and interpreting National policy and of researching best practice, interpreting its relevance and processes/ practices which could be implemented successfully to achieve system reform

PATIENT/CLIENT CARE

  • Oversight of the consistent application of the National Framework and its eligibility decisions for NHS Continuing Healthcare, Children's Continuing Care and Funded Nursing Care.
  • Act as an expert resource in relation to responsible service areas
  • Assist, advise, challenge on the commissioning of high cost and high risk commissioned packages.
  • Oversight and support to the team in relation to legal proceedings (e,g Court Of Protection hearings, Coroners Court).
  • Oversight and support to the team regarding the implementation of PHBs within the locality team.
  • To adhere to N.M.C. professional conduct.

POLICY & SERVICE DEVELOPMENT

To carry responsibility for developing policy and procedure in the area the job holder is currently working on at a point in time, including briefings, commissioning, business or resources.

FINANCIAL & PHYSICAL RESOURCES

  • Responsible for managing the available resources for the relevant work streams, ensuring appropriate allocation of staff resources, identifying departmental pressures and gaps in the current provision and working to provide solutions.
  • Contribute and influence new initiatives at a local and strategic level.
  • Support the provision of financial/workload reports to Directors and the Board, as required.
  • Identifies opportunities to release resources through the development of quality, innovation, productivity and prevention plans.
  • Provide advice and prepare strategic reports and briefings for the Head of the Department when requested.
  • The post holder will be responsible for having an overview of commissioning arrangements and monitor expenditure.

PEOPLE MANAGEMENT

  • Maintaining accurate records in order to keep an up to date account of patients care needs and reduce the risk of errors.

Qulification

Essential

  • In date Nursing and Midwifery registration.
  • Educated to Masters level or equivalent level Experience of working at a senior level in specialist area

Knowledge & Skills

Essential

  • Extensive knowledge of NHS Continuing Healthcare, Children's Continuing Care and Complex health
  • Understanding of the background to and strategic aims of CHC and budgetary impact
  • Significant experience of successfully operating in a politically sensitive environment
  • Sound Knowledge of all NHS Funding Steams for individuals. Mental health capacity Act /DOLs and section 117
  • The ability to influence and negotiate within the local health & social care economy
  • Effective team leadership
  • Identifying innovation & Implementing change
