An exciting opportunity for a Band 4 position exists to join the existing Patient Pathway Facilitator (PPF) team, supporting cancer services. We are seeking a motivated, dynamic, proactive individual to join our team of administration staff that support the Haematology Clinical Specialist Nurses. The team work collaboratively with the Haematology Clinical Nurse Specialist Team to provide a robust pathway for patients diagnosed with a cancer.

The post holder will be required to work autonomously as well as part of the immediate and larger team with clear objectives on supporting the nurse specialists to deliver key priorities in cancer including delivery of Risk Stratified Follow Up Pathways, Holistic Needs Assessment and End of Treatment Summaries.

We are seeking an individual who has a clear vision to meet patient and Trust needs ensuring clinical governance is a priority alongside patient safety.

To achieve these goals, the successful candidate will need to have previous administrative experience including working with databases, spread sheets and managing software systems to ensure the patient pathway is fully supported. An aptitude for problem solving and an eye for detail are crucial.

Main duties of the job

1. To be a point of contact with patients, relatives, carers, and other health and social care professionals using excellent communication skills.

2. Appropriately communicate with patients (over the phone and face to face) to support them and their treatment pathway.

3. Understand the patient treatment and information pathways, assisting patients in accessing resources.

4. To be aware of personal limitations and to seek assistance from other team members as needed.

5. Use initiative and judgement to organise own workload, without close supervision and responding to the requirements of the service.

6. Triage incoming telephone calls, managing onward referrals and sign posting to appropriate person or service.

7. Ensure clear documentation of all interventions made with patient, relative or staff member on relevant internal online systems.

8. Liaise with the team as appropriate and ensure that all non-routine and complex decisions areescalated to the appropriate professional for assessment and review.

9. Communicate with the patients relatives and significant others, involving them in care as appropriate.10. Undertake routine phone calls as required to patients regarding treatment pathways and patient information events, recording outcomes appropriately.

11. Source new and specialist written information for patients.

12. Support patient information production and dissemination to relevant Clinicians and treatment teams in accordance with relevant administration of role.

13. Participate in data audit activity aimed at the provision of quality cancer care as required.

14. To support the collection and maintenance of patient information from a variety of sources, including extracting data from several computerised systems. This will also include liaising with other departments and hospitals to facilitate patient pathways and clinical decisions.

Person Specification

Qualifications

Essential

  • Educated to A Level standard (or qualification equivalent to a Level 3 qualification on the National Qualifications Framework)
  • Demonstrable evidence of continuing professional development
  • Formal IT Training or substantial experience
  • Recognised medical terminology training (or willing to work towards)

Desirable

  • Recognised IMT IM&T qualification (e.g., ECDL)
  • Evidence of recognised customer service training/qualification

Experience

Essential

  • Patient/Customer facing experience within in a clinical/care setting
  • Demonstrable IM&T skills
  • Detailed working knowledge of MS Office applications, specifically Excel and Word
  • Recent experience of working in a customer focused environment, particularly a clinical environment and over the telephone

Desirable

  • Patient/customer facing experience within cancer care
  • Extensive experience of data management/analysis and audit training
  • Experience within cancer services (or similar) administration
