An exciting opportunity has arisen
to join the community nursing service in Halton. Are you an enthusiastic and
motivated Nursing Associate looking for a new challenge? You will join a team
of nurses delivering high quality care to patients who are housebound. This
includes palliative care, wound care, catheter care and phlebotomy. If you feel
you have the right skills to care for patients in their home environment, then
we would like to hear from you. You must be willing to work within an
integrated team and develop your skills and knowledge. Community nursing is
evolving at a fast pace, and you must be able to embrace change.
Main duties of the job
The Nursing Associate will assist in the
identification of health needs and participate in the delivery and evaluation
of care interventions developed to meet those needs under the supervision of a
registered Practitioner. Supervision may be direct/indirect depending upon
competency/complexity of work. The post
holder will continue to develop and undertake a range of delegated tasks and
will report to a Registered Practitioner.

Job description
Job responsibilities
Function as an effective member of the District
Nursing and wider multi-disciplinary team
Maintain clear concise and legible documentation
adhering to prescribed standards and protocols
Use IT systems for relevant tasks e.g., accessing
patient records & results, requesting investigations, data collections
Obtain consent in accordance with Trust Consent
Policy
Use safe handling techniques and promote patient
mobility, utilising mobility aids and equipment where appropriate
To maintain a patient orientated approach towards
care delivery and rehabilitation
Undertake a designated range of patient assessments
(according to protocol)
Plan and deliver high standards of care (in
conjunction with relevant professionals where appropriate)
Implement and evaluate individual care plans,
utilising relevant outcome measures
Undertake a designated range of interventions
(according to protocol) including wound care, application of plaster of paris,
health screening etc.
Recognise the need for referral and undertake
appropriate referrals to specialist nurses/ practitioners/services within the
NHS, independent & voluntary sectors
Promote and provide health education specific to
intermediate care and in line with national and local policies
Undertake, monitor & record the following basic
observations: -
Temperature, Blood pressure, Pulse, Respiration
Peak flow
with readings outside of normal parameters
referred/discussed appropriately within the clinical team
Monitor and record blood glucose levels utilising
blood glucose monitoring equipment, with readings outside of normal parameters
referred/discussed appropriately within the clinical team
Undertake and record urine testing as per Trust
policy, with readings outside of normal parameters referred/discussed
appropriately within the clinical team
Utilise clinical skills appropriately to enhance
the delivery of patient care e.g., Venepuncture, removal clips/sutures
Implement and evaluate a plan of care for simple
wound management, as directed by a registered practitioner
Monitor and care for patients with respiratory
conditions, to include:
Use of oxygen therapy
Positioning to aid
expectoration/breathing

- Competencies achieved within the workplace and attendance at a Higher Education Institute (HEI), as part of the Trainee Nurse Assistant programme.
- Understanding of the limits of the Nursing associate role and responsibilities
