Job summary

The post holder will be responsible for providing a dynamic, high quality clinical pharmacy service to the Care of the Older People services at Whittington Health NHS Trust. They will work closely with colleagues from the Emergency and Integrated Medicine and Adult Community Services team to shape pharmacy services in these directorates and develop new ways of working.

They will facilitate the efficient and cost effective use of medicines within the team, and be responsible for providing a pharmacy service to the Care of the Older People wards and support newer initiatives around medicines management for complex frail older people (including prescribing and pharmacist led clinics, if appropriate).

Main duties of the job

  • To develop, maintain and manage a Clinical Pharmacy service to the Care of Older People
  • To develop, maintain and support Clinical Pharmacy services to patients requiring pharmaceutical support once discharged from the hospital
  • To support Medicines Management activities within the Care of the Older People wards and providing support to the wider Multidisciplinary Team (MDT) in this area, when needed.
  • To support promote good practice over the ICS
  • To work with local care partners (such as GP federations, Primary Care Networksand community pharmacy colleagues)to strengthen and further develop pathways to provide seamless care to our patients based in NCL.
  • Demonstration of expert clinical knowledge, reason and judgement
  • To supportimp lementation of national guidance in medicines management for older people and those with complex pharmaceutical care needs.
  • To act as a leader, mentor and role model to junior staff within Pharmacy.
  • Promote high quality and cost-effective prescribing, along with identifying and promoting best practice.
  • Support the provision of clinical pharmacy working within our current Care of the Older People wards and ensure clinical provision to any new care pathways developed (i.e. Frailty or polypharmacy pathways)

Person Specification

Education and Qualifications

Essential

  • MPharm (or equivalent)
  • Registered with the General Pharmaceutical Council (GPhC) Great Britain or an equivalent EU body
  • Further post graduate qualification Diploma/MSc in Pharmacy Practice or Clinical Pharmacy, or equivalent

Desirable

  • Membership of the Royal Pharmaceutical Society
  • Independent Prescriber
  • Member of relevant professional clinical groups (e.g. UKCPA)
  • Leadership or Management qualification

Skills and Abilities

Essential

  • Good written and oral communication skills.
  • Able to communicate with patients and all grades of healthcare staff (verbally and in writing)
  • Prioritisation and time management, and when under pressure
  • Able to influence and inform decisions about the pharmaceutical management of patients
  • Experience of providing education and training to pharmacy and nursing staff, doctors and other AHPs.
  • Computer literate and able to lead future developments in the use of Information Technology in Medicines Management Good interpersonal skills
  • Confident & assertive when dealing with other professionals
  • Good problem solving skills
  • Demonstrates the ability to communicate complex information combined with good negotiating and presentation skills
  • Ability to communicate with patients particularly on sensitive or complex issues.

Desirable

Knowledge and Experience

Essential

  • Appropriate post qualification experience in the acute NHS sector
  • Previous experience of working within Acute Medicine or Elderly Care
  • Knowledge of national priorities contributing to the development of Frailty and Acute Medicine services
  • Demonstrates awareness of and commitment to the principles of clinical governance
  • Experience of working in a hospital pharmacy department providing a wide range of services.

Desirable

  • Experience of working within Frailty units or Ambulatory Care environments
  • Knowledge of the wider NCL healthcare system and priorities
  • Knowledge of change management principles
  • Experience of contributing or leading quality improvement projects
  • Previous participation in audit and / or research.
  • Understanding of health service issues in primary and secondary care.
  • Experience of identifying cost saving opportunities
  • Experience of line management and / or team leadership
  • Experience of MDT working
  • Experience of development of clinical policies and guidelines
