Job summary

The Children's Continuing Care team are looking for an enthusiastic registered nurse, with a strong clinical background with supporting children with complex health care needs to join our team. The successful candidate will need to have a passion for working with children and their families, whilst helping to promote independence and wellbeing for children and young people with complex care needs.

Each day working in the continuing care team is different, and our team members are always looking for ways to overcome the issues that we face, so the successful candidate must enjoy a challenge, be a dynamic thinker and problem solver.

The successful candidate will be part of the clinical team, working independently managing their own case load and collaboratively with other key professionals whilst ensuring that they fulfil the ethos of looking after the whole child. They will have an opportunity to build cross agency relationships, whilst working closely with colleagues from Social care and Special Education Needs.

Working closely with our admin team the successful candidate will plan and coordinate their own MDT meetings, including support the arrangement of emergency provision for children who are fast tracked to the team due to needing end of life care or experiencing mental health crisis.

The successful candidate will participate in completing assessments as part of the ICB delivery of the NHS Continuing Care framework.

Job responsibilities

The Continuing Care Team works across the boroughs of Camden, Islington and Haringey consists of both qualified and associate care staff. The team co-ordinates and provides direct care into the homes of children who have complex life threatening/ life limiting conditions. These children have a range of conditions, some requiring ventilation via BiPAP/ CPAP. They require support both in and out of the home, some of them attending school.

The post holder will be responsible for supporting the facilitation and organisation of care, as well as some direct care, for children and young people with continuing care health needs. They will be responsible for the effective assessing, planning, implementation, delivery, and evaluation of individualised nursing care to children as required and under the direction of the Senior Practitioner. The post holder will develop skills and knowledge in the use of the National Continuing Care Framework and participate in care assessments and reviews.

The post holder will support training and development of a team of carers to ensure all aspects of care are delivered safely to an allocated caseload. The post holder will support the Senior Care Team to continue the strategic development of the service.

The post holder will act up in the absence of the Senior Practitioner for the Continuing Care team; with excellent leadership and development opportunities.

MAIN DUTIES

  • Provide direct care to children with complex health needs in the home setting.
  • To train and develop Care staff through competencies to ensure they are sufficiently skilled to care for specific children on the complex care caseload.
  • To assist the senior practitioner in the supervision, teaching and supporting of a team of associate carers that provide health care support for children dependent on technology.
  • Supporting the Senior Practitioner in the review of care plans and outcomes, contributing to the regular reports for managers and commissioners.
  • Involve parents and young people in service review.
  • To attend staff and other relevant meetings/team days.
  • To demonstrate a commitment to implement elements of clinical governance and to contribute to improving standards of care in line with recommendations
  • To participate in research projects and /or innovations, including Quality Initiatives
  • To participate in the audit process for monitoring and reviewing quality.
  • To maintain up-to-date, accurate records, input work data into the computer and to provide written reports as requested
  • To be aware of and act in accordance with agreed policy procedures legislation and NMC standards.

Person Specification

Education/Qualifications

Essential

  • RN Child or RSCN at degree level
  • NMC registration
  • Evidence of Continued Professional Development post Registration

Desirable

  • High Dependency Nursing Course

Knowledge and Experience

Essential

  • Experience of working in paediatric inpatient /ambulatory care / community or paediatric A&E
  • Knowledge and understanding of the Continuing care framework
  • Ability to work without supervision
  • To be competent and experienced in ventilation and facial CPAP/BIPAP
  • Experience of acting as a mentor to junior staff/student nurses

Desirable

  • Experience of working within a community setting with complex needs children and young people
  • To be competent and experienced in tracheostomy
