Main duties of the job
As an Advanced
Nurse Practitioner (ANP) in a General Practice/Primary Care setting, you will
provide advanced nursing care, assessment, and treatment to patients of all age
groups. In addition to working within the practice setting, you would undertake
home visits to patients who are unable to attend the practice due to mobility
issues, chronic illness, or other medical reasons. The ANP will collaborate
closely with the healthcare team to ensure comprehensive and patient-centered
care.

Our South Southwark PCN is made up of
all GP Practices in four neighbourhoods - Camberwell, Peckham, Nunhead and
Dulwich, serving approximately 150,000 patients, supported by their GP
Federation, Improving Health (IHL).
The NHS Long Term Plan describes the
prominent role Primary Care Networks play in delivering proactive, personalised
and more integrated health and social care for their local populations. This
will require collaborative working between organisations including GP
practices, acute and community health, social care organisations and the
voluntary and community sector.

1. MAIN
DUTIES AND RESPONSIBILITIES
Patient
Care and Assessment:
Conduct
thorough assessments of patients' health conditions, including physical
examinations, medical histories, and symptom evaluation.
Order
and interpret diagnostic tests and investigations.
Develop
and implement evidence-based treatment plans for patients, including
prescribing medications, administering treatments, and providing health
education.
Collaboration
and Consultation:
Collaborate
with the general practitioner (GP) and other healthcare professionals to manage
complex or challenging cases.
Provide
consultations to patients, offering advice, information, and support to promote
self-management of conditions.
Participate
in multidisciplinary team meetings, contributing expertise and insights to
optimize patient outcomes.
Health
Promotion and Education:
Educate
patients and their families about health promotion, disease prevention, and
self-care techniques.
Provide
guidance on managing chronic conditions and promoting a healthy lifestyle.
Offer
counselling and support for emotional well-being, mental health concerns, and
lifestyle modifications.
Documentation
and Record-Keeping:
Maintain
accurate and up-to-date electronic health records for all patient interactions,
including home visits.
Ensure
adherence to data protection and patient confidentiality regulations.
Document
interventions, treatment plans, and outcomes to facilitate effective
communication and continuity of care.
Professional
Development and Quality Improvement:
Stay
updated with the latest evidence-based organisations, guidelines, and
regulatory requirements in primary care.
Engage
in continuous professional development activities, such as attending
conferences, workshops, and relevant training programs.
Contribute
to quality improvement initiatives within the organisation, suggesting and
implementing strategies to enhance patient care.
Person Specification
Qualifications
Essential
- Registered Nurse (RN) qualification with active registration with the Nursing and Midwifery Council (NMC)
- Advanced Nurse Practitioner qualification (ANP) at master's level or equivalent.
- Additional qualification or certification in Independent Prescribing (V300) or willingness to obtain this qualification.
- Proven experience as an Advanced Nurse Practitioner in a General Practice/Primary Care setting.
- Demonstrated competence in conducting home visits and managing patients in their own homes.
- Excellent clinical assessment, diagnostic, and decision-making skills.
- Strong communication and interpersonal skills to establish rapport with patients, families, and multidisciplinary teams
