Main duties of the job

It is essential to us that all people being assessed receive an equal and comparable, good quality, consistent, high standard; person-centred experience no matter who they are or where they are or when the assessment takes place.

Your role as a Clinical Manager within the Children and Young Persons division of All Age Continuing Care (AACC) will be key to ensuring that we are able to implement our above aim in a high quality way.

As an AACC Clinical Manager, you will work under the guidance and support of the senior team leads to implement the National Service Framework for NHS Children and Young Persons Continuing Healthcare across Mid and South Essex.

You will promote best practice and provide professional advice to parents, carers, residential provisions and other providers, promote good relationships and be able to liaise with colleagues from other disciplines both internally and externally to the Integrated Care Board (ICB).

The ideal candidate will have excellent communication and children and young person assessment skills. They will be able to work proactively to problem solve and prioritise their workload. This will include the case management of patients with complex health needs. Successful candidates will need to have a flexible attitude and be a keen team player who enjoys working within an energetic fast-paced environment. ROLE PURPOSE / SUMMARY

The role of the Mid and South Essex Integrated Care Board (MSE ICB) is to join up health and care services, improve peoples health and wellbeing and reduce health inequalities for our 1.2m population through our wider system and four Alliances. MSE ICB are committed to place clinical and professional leadership and the voice of our residents at the heart of realising all its ambitions.

This role sits within the Nursing and Quality Directorate, in the All-Age Continuing Care (AACC) service, which incorporates the Continuing Healthcare (CHC), Children and Young Peoples Continuing Care (CYPCC) teams and the Neuro Navigation team.

The AACC service delivers the statutory duties of the ICB to assess and to provide for services to meet the identified needs of those found eligible for Continuing Healthcare. The service delivers that duty through the implementation of the National Framework for Continuing Healthcare and NHS Funded Nursing Care (2018) and the National Framework for Children and Young Peoples Continuing Care (2016), working in accordance with the AACC Operational Policy.

The Clinical Support Manager role will provide professional leadership, advice and expertise to ensure delivery of a high-quality standard of clinical assessment, review and management of AACC cases, working collaboratively with key partners to achieve best outcomes. The post holder will provide advice, support, supervision and line-management to the Clinical Support Officers, ensuring that they deliver the highest quality and most appropriate services to patients whilst driving continuous service improvement.

COMPETENCY BASED KEY DELIVERABLES

Dependent on allocated caseload, the role may include:

  • To provide expert advice and guidance to support both health and social care staff who are involved with patient assessment, care provision and care and support planning.
  • To provide assessment, care and support planning, ongoing case management and review for all patients eligible for NHS continuing care funding.
  • To assess retrospective claims for CHC eligibility.
  • To support panels, appeals and local resolution processes as clinical representative.
  • To undertake or coordinate appropriate assessments, approvals and notifications of deprivations of liberty, maintaining comprehensive records.
  • To ensure the agreed care and support package continues to meet the individuals assessed health and associated care/support needs and their agreed outcomes.
  • Responsible for decisions made during the course of duties, seeking guidance and escalating issues where appropriate.
  • Contribute to the delivery of one or more strategic plans, enabling key milestones to be delivered and raising any issues associated with the delivery of plans and workstreams.
  • Management of a small team.
  • Be financially aware of AACC spend and ensure best value for money in service offer
  • Can demonstrate and deliver business acumen in their area of responsibility ensuring value for money, understanding the business of partners and the driving factors for delivery in multiple sectors.

  • Substantial experience of working at a senior clinical level in AACC services.
  • Experience of working in the NHS/public body
  • Experience of working in a service focused environment.
  • Experience of holding responsibility for budget/s, budget setting and working knowledge of financial processes within own service
  • Experience of leading, managing and delivering projects involving stakeholders
  • Experience of managing incoming and outgoing information requirements, handling correspondence/queries which may be highly complex, sensitive, or highly confidential.
  • Experience of developing policies/practices/systems
  • Demonstrable line management experience
