Job summary

Arden PCN are looking to recruit a Clinical Pharmacist who is passionate for delivering a quality service to their patients. The Clinical Pharmacist role supports to improve patients health outcomes and of the efficiency of the primary care team by providing direct, accessible and timely medicines expertise, including face to face. Clinical Pharmacist proactively transfer workload relating to medicines optimisation issues to improve patients care safety and free up clinical staff to spend more time on clinical care.

Location:The successful candidate will be based at Alcester Health Centre, but must be willing to travel across all practices if required to support business needs. Due to the geography and nature of the role a clean UK driving license and full use of car is essential.

Main duties of the job

The post holder will help patients to get the best from their medicines by switching medications to agreed and approved protocols, improving repeat prescribing processes in General Practice, including promotion of repeat dispensing and online ordering, minimising clinical risk and aiming to reduce wasted medicines.

  • To work as part of a multi-disciplinary team in a patient-facing role to clinically assess and treat patients using extensive experience and in-depth knowledge to be directly responsible for care and prescribing of defined cohort(s) of patients
  • To deliver and develop pharmacy led medicines optimisation working with general practice and the wider multidisciplinary team, ensuring practice remains up to date
  • To lead and promote, support and facilitate medicines optimisation for specialist area in the wider medicines agenda ensuring best value, reduced harm and improved outcomes.
  • To lead and deliver evaluation and audit of service delivery for specialist area in order to ensure a high-quality service and to further develop provision.
  • To work to the principles of Medicines Optimisation and relevant up to date national guidelines and evidence base.

Job responsibilities

To improve patients health outcomes and the efficiency of the Primary Care team by providing direct, accessible and timely medicines expertise including face to face. To proactively transfer workload relating to medicines optimisation issues to improve patients care, safety and free up clinical staff to spend more time on clinical care.

Provide proactive leadership on medicines and prescribing systems to the PCN multidisciplinary team, patients and their carers.

Develop and deliver patient facing services and clinics which improve patients outcomes, reflect the needs of the PCN population and shift workload from other disciplines.

Deliver direct patient consultations and prescribing as part of the practice and cluster multidisciplinary team approaches acute illness.

Improve patient and carer understanding of, confidence in and compliance with their medicines.

Provide pharmaceutical consultations and home visits to patients with long term conditions as an integral part of the multidisciplinary team.

Field medicines and prescribing queries from patients and staff.

Organize and oversee the PCNS medicines optimisation system including the repeat prescribing and medication review systems.

Deliver high quality medication review and develop bespoke medicines treatment plans for patients (particularly the elderly, complex patients, and those at high risk of admission)

Improve the safety and quality of prescribing following hospital admissions and attendance

Improve the quality and effectiveness of prescribing through clinical audit and education to improve performance against NICE standards, prescribing guidance, and other clinical standards.

Develop yourself and the role through participation in clinical supervision, training, and service redesign activities.

Provide a clear and active link with community pharmacists and other stakeholders in the medicines supply chain to improve patient experience and manage issue.

Deliver training, monitoring and guidance to other clinicians and staff on medicines issues.

Contribute to the achievement and maintenance of good to outstanding CQC registration status.

Person Specification

Qualifications

Essential

  • GPHC Independent Prescribing Qualification.
  • The successful applicant will have completed the CPPE pathway, or be at least half-way through.
  • Evidence of post qualifying and continuing professional
  • development.
  • Extensive knowledge of medicines optimisation through post graduate diploma or equivalent experience or training.
  • Member of GPHC.

Experience

Essential

  • Must understand the background to and aims of general practice.
  • Has appreciation of new NHS architecture including the relationship between individual provider, PCN and commissioners.
  • Understanding of the current national policies those impacting on medicines optimisation services.
  • Ability to constructively challenge the views and practices of clinicians and managers.
  • Proven verbal and non-verbal communication skills.
  • Good interpersonal skills.
  • Considers most effective way to promote equality of opportunity and good working relationships in employment and service delivery; can take actions which support and promote this agenda.
  • Ability to travel round the PCN GP Practices.
  • Embrace change, viewing it as an opportunity to learn
  • and develop.

Desirable

  • Good understanding of primary care.

  • Evidence of experience in using independent prescribing.

Skills & abilities

Essential

  • Demonstrates understanding of legal, governance and ethical frameworks within which pharmacists and primary/secondary.
  • Evidence of planning and delivering of programmes

  • Ability to summarise and present information in relevant concise and

  • Current and valid UK driving license with access to a car.
  • Excellent clinical knowledge and ability to apply to
  • practice.
  • Experience of a patient focused role and consultation skills to apply knowledge in a format patients/carer can understand.
  • Sound knowledge of medicines legislation and the vision for the development of clinical pharmacists within primary care.
  • Understanding of the data protection act and patient confidentiality, clinical governance, information.
  • Able to understand broad range of complex information quickly and
  • make decisions.
  • Working knowledge of Microsoft Office with intermediate keyboard skills.
  • Ability to work without supervision, providing specialist advice to the PCN, working to tight and often changing timescales.
  • Interpreting national policy for implementation.

  • Ability to critically analyse complex information and evidence and communicate the findings clearly in simple terms.
  • Ability to identify risks, anticipate issues and create solutions and to resolve problems in relation to project or service delivery.
