Job summary

We are looking to recruit a compassionate and dynamic fully qualified Advanced Clinical Practitioner to join the team at the Arthur Medical Centre at Horsley Woodhouse.

This is a great opportunity for an enthusiastic ACP to lead the care of a frail and elderly population with the aim of keeping older people well at home for longer and reducing hospital admissions.

The post holder will also proactively manage the health needs of care home residents through regular ward rounds, anticipatory care planning, medicines optimisation and long-term condition management.

The successful candidate will have completed an MSc in Advanced Practice (or equivalent) and will be able to demonstrate contemporary experience of working at an advanced level.

The practice prioritises education and development so is looking for a self-motivated, enthusiastic individual with a passion for continued learning.

Main duties of the job

You will apply advanced clinical expertise, levels of judgement, discretion and decision making to the provision of clinical care to older people across primary and community care settings.

You will exercise a high degree of autonomy through the application of advanced clinical skills in relation to the assessment, diagnosis, treatment and management of patients, in particular the older person.

You will lead and monitor improvements to quality of care through clinical audit, evidence-based practice, clinical supervision, teaching and supporting professional colleagues and the provision of skilled professional leadership.

You will lead the implementation of relevant clinical guidelines into practice and contribute to the further development of the evidence base through research and audit.

Education and Qualifications

Essential

  • Professional registration with NMC or HCPC
  • Evidence of post-registration study at undergraduate level (Level 6)
  • Teaching qualification
  • MSc in Advanced Clinical Practice (or equivalent)
  • Independent Prescribing or study of pharmacology & therapeutics at post-graduate level (Level 7)

Experience and Knowledge

Essential

  • Previous clinical experience with experience at a senior level
  • Contemporary clinical experience of older people with complex needs
  • Experienced mentor, appraiser or clinical supervisor
  • Participating in a multi-professional team
  • Clinical expertise in end of life care
  • Practical understanding of primary care and community services
  • Involvement in service improvement
  • Implementation of best practice
