Job summary
Central Camden PCN are a group
of 9 member practices who work together at scale and collaborate with others in
the local health and social care system. The PCN is highly innovative and
delivers services with support from the GP Federation, Camden Health Evolution
(CHE) Ltd, that aim to improve the quality of care and outcomes for their
patients. We are looking to recruit a Social Prescriber to work within our
Primary Care Network multidisciplinary healthcare team.
Main duties of the job
- Proactively identify and work with people, including the frail/elderly and those with long-term conditions, to coordinate and navigate care and support across health and care services.
- Work closely with member GPs practices and PCN workforce to ensure appropriate support is available, helping patients understand and manage their conditions, and addressing their changing needs.
- Enable people to access the services and support they require to meet their health and wellbeing needs, improving their quality of life.
- Work alongside the member GP practice teams to provide a comprehensive approach to personalized care and help people navigate the health and care system.
- Engage with a diverse range of people from different cultural and social backgrounds, working confidently and effectively in varied, sometimes challenging environments.
- Possess excellent interpersonal and communication skills, and be organized, patient, and empathetic, with experience in health, social care, or other support roles, including direct contact with people, families, or carers.

Job description
Job responsibilities
Social Prescribers play a crucial role within our
Primary Care Network (PCN), identifying and working with people, including the
frail/elderly and those with long-term conditions, to coordinate and navigate
care and support across health and care services.
Social prescribing empowers people to take control
of their health and wellbeing through referral to link workers who focus on
what matters to them and take a holistic approach to their health and
wellbeing. Link workers connect people to diverse community groups and
statutory services for practical and emotional support, supporting existing
groups to be accessible and sustainable, and helping people start new community
groups.
Social Prescribers work as a key part of the PCN
multi-disciplinary team, helping to strengthen community and personal
resilience and reduce health and wellbeing inequalities. They address the wider
determinants of health, such as debt, poor housing, and physical inactivity, by
increasing peoples active involvement with their local communities.
The successful candidate will manage a caseload of
clients through assessment to onward referral, working with clients referred by
the member GP practices and Anticipatory care service, and providing ongoing
support to promote engagement with identified services and achievement of goals
The Social Prescriber will be an integral part of
our PCN anticipatory clinic, working closely with the clinical team to conduct
MDT reviews on our most vulnerable patients.
This role will be key in delivering the NCL Long-term
condition LCS, which places an emphasis on shared decision-making and a
holistic personalised care approach to chronic disease management.

Essential
- Essential:
- Experience working in health, social care, or other support roles with direct contact with people, families, or carers (paid or voluntary).
- Experience supporting people, their families, and carers in a related role.
- Strong organizational and written skills, including planning, prioritizing, time management, and record-keeping.

- Desirable:
- Previous experience working as a PCN based Social Prescriber
- Knowledge of Safeguarding Children and Vulnerable Adults policies and processes.
- Disclosure and Barring Service Check.
