Northumberland Head Injuries service are happy to be offering an exciting opportunity for Band 6 Community Practitioner. We would welcome applications from Social Workers, Nurses or AHP's

We are a multidisciplinary community team working across the whole of Northumberland with individuals with complex neurological needs following Traumatic Brain Injury (TBI). The successful post holder will be expected to demonstrate a strong commitment to high quality patient care, good organisational skills and be able to demonstrate prioritisation of workload. Key components of the role will be to ensure the service offers a safe, caring, responsive and well-led service to the people we serve.

The role will include liaising with agencies such as social care, service providers, inpatient services, safeguarding, alongside a range of other agencies. Within this team we pride ourselves on having a broad range of knowledge within these selected areas and ability to manage the risks associated with this.

Main duties of the job

To be responsible for the assessment and management of a caseload of patients who have complex neurological needs following Traumatic Brain Injury.

Provide specialist clinical advice to patients, families, carers, staff, statutory agencies, third sector partner agencies and the public.

You will be expected to provide high quality assessment, working collaboratively with Patients, Carers and the MDT.

Work collaboratively with patients, carers and the MDT to develop individual neuro rehab goal plans, ensuring patients have access to regular reviews to monitor progress and adapt the rehab plans as necessary.

To consider carer needs and ensure carer and family are involved in care planning where appropriate.

To deliver evidenced based clinical interventions.

The post holder will work as a member of the Multi-disciplinary Team ensuring that high quality individualised care is delivered which ensures safety and a commitment to promoting recovery and wellbeing and maximising independence.

Coordinate treatment planning, interventions, clinical reviews and discharge planning.

Provide clinical supervision to junior staff and students as delegated by Clinical Lead.

To provide compassionate care that is based on empathy, kindness, respect and dignity.

To incorporate a patient centred philosophy into practice and patient centred collaborative care.

Person Specification

Education and qualification

Essential

  • Professional Qualification and current Professional Registration as, a First Level Registered Nurse (RNMH, RMN, RNLD) or equivalent as an AHP, Social Worker or Clinical Psychologist
  • Evidence of post-registration training in clinical interventions/skills relevant to the post.
  • 12-24 months minimum post registration experience

Desirable

  • Experience of working in a community team.

Skills and Competencies

Essential

  • Proven ability to carry out and provide comprehensive written assessment of individuals with complex health needs.
  • Risk Assessment and risk management/safety/planning skills
  • Demonstrate an understanding of effective and appropriate treatments options including a range of experience based clinical approaches.
