Our Team is an ever-growing community that provides services from Cystic Fibrosis, Play Therapy, CYP Continuing Care, Palliative Care and Transition.

Main duties of the job

To promote and sustain the independence of children & young people (CYP) with a range of complex healthcare needs by delivering skilled nursing care within the community in a variety of settings.

To undertake assessments of care, develop, implement and evaluate care programmes, in partnership with the child, young person and their family. To carry out managerial responsibilities for an identified caseload and skill mix of staff.

Job responsibilities

To assess, plan and implement care to meet the identified individual healthcare needs of CYP, and families (this will include provision of continuingcare support).To work in partnership with other care providers to facilitate, a prompt andefficient discharge from hospital, subsequently reducing hospital admissionsand unnecessary re-admissions.Working in partnership with the family, Consultant Paediatricians, NurseConsultants and key agencies, from both the statutory and voluntary sector.To participate in the risk analysis and decision making process at childprotection meetings, providing written reports and giving health advicewhere needed.Initiate and develop flexible and innovative approaches to meet the needs ofCYP and their families.Following a period of induction to participate in telephone out of hoursadvise service and 24/7 support when delivering an episode of activepalliative care on call.To maintain a professional attitude at all times, developing the role withinthe Community Childrens Nursing Team, (CCNT) in the best interests ofthe profession and in accordance with the NMC Code of professionalconduct. Also providing on-going evidence of eligibility to practice.To assess and identify equipment which is suitable for CYP and familyneeds following the medical devices and procurement policy.

To be flexible to work within a 24 hour shift pattern, including weekends and bank holidays.Knowledge skills and experience required:Participate in Induction, training and preceptorship programmes for qualifiednursing staff, including student nurses, healthcare support workers and asagreed with other agencies.To participate in relevant nursing forums to establish links.To be actively involved in the development of standards, protocols, carepathways and clinical audit.To be in involved in providing training to enable staff from multidisciplinary &multiagency partners to meet agreed competency levels.To provide clinical supervision and support to staff, one to one and groupsupervision and carry out personal development reviews as required.Develop own and others knowledge and practice and organisationalboundaries by providing clinical advice as required.Contribute to monitoring quality and the investigation of incidents.

Update and maintain professional knowledge in relation to your area ofinterest around the national and local modernisation agenda for children,young people and their families.To be aware of own learning needs and limitations and to maintainprofessional expertise by embracing the concept of lifelong learning.To work alongside the clinical lead to support the operational running of theCCNT. (e.g. Sickness return to work interviews etc.)Measurable Results areasTo undertake risk assessments, complete the necessary documentationand take appropriate action including informing your line manager.Utilise all resources efficiently and effectively within the locality.The post holder will maintain accurate and contemporaneous records, dealwith highly sensitive information respecting confidentiality and security at alltimes in accordance with Trust policies and data protection.To be involved with and/or support staff in auditing services and carepractices on clearly identified areas as per Trust audit guidelines.To ensure that all clinical incidents and national patient safety alerts arereported, actioned and reviewed in a timely manner.To be responsible for and provide evidence that all mandatory training isundertaken & up to date as per Trust training matrix.The post holder will be required to use a computer, either stand alone or aspart of a networked system, and will be responsible for the quality ofinformation. The amount of time spent on this type of work will depend onthe current needs of the caseload.Communications and Working Relationships:To ensure CYP obtain information and optimal treatment, taking intoaccount social, cultural norms.To develop and maintain effective communication networks with serviceusers, to support the enhancement of service delivery.To develop and maintain an effective and comprehensive communicationnetwork within integrated childrens services (health, social care, education,acute care, hospices and the voluntary sector) and other external agenciesto ensure the best service for CYPs and their families.To promote and advocate for the equality, diversity and rights of children &young people who access the service.Demonstrate sensitivity towards CYP and their families by respecting theprivacy of their home environment and maintaining professional boundariesat all times.

Qualifications

Essential

  • Current NMC Registration
  • Recognised teaching qualification

Desirable

  • First level degree/specialist practitioner equivalent experience or willingness to work towards degree level (appropriate role)
  • Non -Medical Prescribing qualification or willingness to undertake.

Experience

  • Extensive post registration experience.
  • Evidence of recent continuing professional development
  • Experience of caring for children with complex and palliative care needs.
  • Evidence of multi-disciplinary working

Desirable

  • Minimum 12-18 months working at Band 5 level in the care of sick children.
