Job summary

Siam Surgery has a fantastic opportunity for an experienced nurse practitioner/advanced nurse practitioner to join our friendly, hardworking team on a full-time or part-time basis, hours negotiable.

Main duties of the job

  • Delivering a high standard of patient care, using advanced, autonomous clinical skills and a broad, in-depth, theoretical knowledge-base
  • Completing both face to face and telephone consultations
  • Conducting home visits
  • Triage
  • Working collaboratively with the multi-disciplinary general practice team
  • Providing expert advice to patients, carers and colleagues
  • Developing innovative concepts, models, methods and practices, to help deliver improved primary care to our practice population
  • Advancing patient health and well-being through the use of health promotion, health education, screening and therapeutic communication skills
  • As an independent prescriber: to prescribe safe, effective and appropriate medication, as defined by current legislative framework
  • Leading and undertaking research and audits
  • Providing leadership, education and training to different healthcare professionals, staff and students
  • As a vital part of our multidisciplinary team, you will have the opportunity to support and care for a diverse patient group.

About us

Siam Surgery is a semi-rural GMS practice which provides a high level of care to just over 12,000 patients in Sudbury, Suffolk and its surrounding villages. We are a well-established, friendly and supportive practice with a growing team.

The Practice:

  • Stable partnership of 3 GPs supported by a dedicated team of 4 nurse practitioners, 3 practice nurses and a trainee nursing associate.
  • Large multi skilled non-clinical team.
  • Part of a 2 practice PCN with an excellent team of multidisciplinary staff integrated into the practice (first contact physio, paramedic, mental health practitioners, clinical pharmacists, social prescribers and care coordinators).
  • SystmOne clinical system.
  • Rated good in all areas by CQC.

Person Specification

Experience

Essential

  • NMC registered.
  • Advanced clinical assessment and examination skills.
  • Management of patients with long-term health conditions.
  • Management of patients with complex physical/psychological/social needs.
  • Management of palliative and end of life patients.
  • Experience as a nurse practitioner in general practice.
  • Nurse-led management of minor illness, minor ailments and injuries.
  • Reliable.
  • Forward-thinking.
  • Adaptable.
  • Passionate about patient care.
  • Highly motivated.

Desirable

  • Experience of SystmOne and ARDENS is desirable but not essential.

Qualifications

Essential

  • MSc/Degree in Advanced Nursing or RCN accreditation of Masters level study
  • Independent prescribing qualification
