Job summary

This is an exciting opportunity to join our dynamic and enthusiastic team as a Junior Clinical Fellow (F3), who believe in leading from the frontline, with the aim of providing outstanding care to our community and investing in thewell-being and development of our people.

The "Foundation Year 3" post will include the opportunity to develop a special interest in each placement. This post is designed to cover training post gaps or established trust posts.

Main duties of the job

The FY3 post in Neurology will involve daytime ward-based and outpatient work only. Thesuccessful applicant will work daily with the on-site consultant neurologist and specialist registrar to provide specialist neurology opinions for patients admitted to the RBH. In addition, the appointee will have responsibility for managing outpatients attending the Battle Day Unit for infusion therapies such as intravenous immunoglobulin and disease-modifying therapies for multiple sclerosis; and for initiating steroid-sparing agents in for outpatients with myasthenia gravis and other autoimmune conditions. Training will be provided in performing lumbar punctures and then appointee will have a regular outpatient list for this procedure. Outpatient exposure will be provided in the form of a weekly urgent neurology clinic under the direct supervision of a consultant neurologist. Attendance of the weekly neuro-radiology / neuro-oncology MDT meeting would be expected.

Opportunities to observe or participate in sub-specialist activities such as botulinum toxin injection for movement disorders; apomorphine and foslevodopa/foscarbidopa infusions for Parkinson's disease; specialist PD, MS, stroke or epilepsy clinics; nerve conduction studies, EMG and EEG may become available during the course of the post.

Weekly attendance of the RBH medical grand round will be timetabled, and ad hoc attendance of the Oxford neurology grand round will be encouraged, other duties permitting.

About us

The RBH neurology service is staffed by five consultant neurologists, one associate specialist, and two consultants in neurological rehabilitation. Each neurology consultant is based in Reading but has a weekly visiting specialist role at the John Radcliffe hospital in Oxford. We provide a highly-responsive 24/7 on-call service for inpatients, and also participate fully in the acute stroke rota. There is also a visiting specialist registrar on the Oxford rotation who is on-site Monday to Thursday. At present there are no inpatient neurology beds at the RBH but we are closely involved with the care of patients with neurological issues throughout their hospital stay. There is a 14-bed medium-stay neuro-rehabilitation unit at West Berks community hospital.

We are able to offer a number of services in Reading which are often reserved to teaching hospitals such as a DMT service for MS patients; advanced therapies for Parkinson's disease; subspecialist opinions for epilepsy, stroke and Huntingdon's disease patients; nerve conduction and EMG (and EEG in the near future). We work closely with colleagues in the elderly care, stroke medicine and paediatric departments to deliver long-term condition follow-up. We also have a large team of nurse specialists for MS, PD, epilepsy and rare disorders lead by a nurse consultant.

Post holders are entitled to 5 days study leave per 6 months and can attend trust teaching appropriate to their grade (co-ordinated a department ensuring adequate ward staffing cover). We are in the process of creating a dedicated monthly teaching programme for the Foundation Year 3 cohort.

We are determined to support successful applicants with career progression and actively encourage and support involvement in Quality Improvement, learning and exam/interview preparation. The Foundation Year 3 Doctors now have a dedicated Clinical Tutor to ensure there is an educational aspect to the posts.

The post holder will be assigned an educational supervisor for the duration period and will be encouraged to attend appropriate teaching sessions within the trust, including the potential of sub- specialist exposure to support career progression and applications to training programmes.

We also run a structured teaching programme in which FY3 doctors provide medical student and physician associate teaching and obtain feedback on this. FY3 doctors also have the option to be mentored by previous FY3 doctors. We ask a current FY3 Doctor to be a representative to the Junior Doctors Forum, to be a conduit for training and educational concerns from themselves, or those Doctors in training posts.

  • MBBS or equivalent medical qualification
  • Advanced Life Support certification

Experience

Essential

  • Completion of Foundation year 2.
  • Previous experience of working in the NHS as a doctor.

Desirable

  • Previous experience in one of the following specialities: o Stroke, neurology or other medical specialities at FY2 or higher level o Intensive Care medicine o Emergency medicine
  • Post registration experience working in other specialties relevant to the role.
  • Provider status in one of the following: ALS, ATLS
