West Hertfordshire In-patient Service: B5 Physiotherapist
An exciting opportunity has arisen within our in-patient Intermediate care general rehabilitation service at West Hertfordshire. We are looking for a band 5 Physiotherapist to join the Physiotherapy team and wider MDT at Oakmere Ward, Potters Bar Community Hospital, for a 1 year fixed term contract.
This is a service for the rehabilitation of adults presenting with a range of physical dysfunction/or disabilities resulting from injury, illness, surgery or chronic disease. The majority of the patients are referred from an acute hospital and some may be referred from the community settings and establishments. We are looking for a highly motivated individual to join the team, who has the passion and skills for delivering, and maintaining in the team, a high standard of rehabilitation. You will be working with a multidisciplinary team that includes Occupational Therapists, Nurses, Dietitian, Rehabilitation Assistants, Speech and Language therapists and Social Workers. There is a potential to turn this role in to a rotational post in the near future.

Job description
Job responsibilities
To assess the functional needs of patients providing intervention plans aimed at improving function and reducing the impact of disability, working with patients to identify Physiotherapy goals as part of their overall care plan.
To develop and implement treatment plans for patients referred to the services. These patients may present with a variety of multi-pathologies within specialist areas, including neurological conditions, musculoskeletal conditions, age related disorders, osteoporosis, learning disability and/or mental health diagnosis, with support from Senior Therapists.
To undertake comprehensive assessment, planning and treatment of patients using clinical reasoning, in order to implement therapeutic intervention.
To communicate relevant medical information, assessment details, advice, recommendations and discharge plans across agencies and with clients in compliance with Trust Guidelines for sharing of information and informed consent. These agencies may include General Practitioners, Social Services, Healthcare professionals, voluntary agencies, Residential and Nursing homes.

Education/Qualifications
Essential
- Diploma/degree in Physiotherapy
- Current HPC registration
- Evidence of up to date CPD portfolio.
Desirable
- Member of CSP
- Relevant Post Graduate Training
