What does the Community Response Team do?
They step in to support people who are discharged from acute hospitals and need short term/interim at their homes. The teams are committed to caring and supporting people at home and work over a seven day rota.

What are the hours?
The shift pattern is Early-0800-1400 and Late 1600-2200 over a seven day rota and we have full time hours available (36 hours) along with part time options minimum of 18 hours per week.
Where would I be working?
Base: Walton Hospital, Chesterfield
Area Covered: Chesterfield/North East Derbyshire
Working in this team means you will need be able to meet the travel requirements of going to people's homes. This will require you have a UK driving license and access to a vehicle. DCHS has a mileage policy and car lease scheme available to support our staff.
Main duties of the job
The successful candidate is expected to work and document to a high standard and work in a way that reflects the DCHS values.
To maintain standards, ensuring quality of patient care through the development of a range of knowledge and competencies to support clinicians.
To assist all clinicians in delivering agreed programmes of treatment and care where appropriate and work as an effective member of the wider multi-disciplinary team.
To carry out individual care without direct supervision in the community.
To provide support to patients to optimise their health and independence.

Education, Qualifications and Training
Essential
- NVQ Level III in care / BTEC Health & Social Care or equivalent
- Experience of working health care setting

Experience and Knowledge Required

- Working in health and or social care environment
- Understanding of factors that influence health and wellbeing
- Understanding of the implications for, and responsibilities of the post for risk management and safeguarding
