Job summary

gtd healthcare is one of the largest NHS commissioned Primary Care, Out of Hours & Urgent Care providers in the North West of England.

We have fantastic opportunities for Advanced Clinical Practitioners to work in the Out of Hours, Digital Hub and Urgent Care Teams in Oldham, Denton and Central Manchester within a single service location or portfolio role.

Flexible full time and part time positions are available for Advanced Clinical Practitioners with In-hours, out of hours, evening, weekend and overnight working hours available.

Whilst working as an Advanced Clinical Practitioner, you will work alongside a supportive multidisciplinary team that includes Nursing, Allied Health Professionals and GPs, who have a real commitment to continuous improvement, shared learning and best practice, while being passionate about providing patients with the highest possible quality care.

Main duties of the job

The post holder will hold an MSc in Advanced Clinical Practice or Credentialing qualification (or equivalent) and a Non-Medical Prescribing qualification. You will be able to work within the four pillars of practice.

Within your professional boundaries you will assess, diagnose and prescribe where appropriate. You will provide advice, care and management patients presenting with undiagnosed or undiagnosed undifferentiated illness. Your role will include supporting the delivery of policy, procedures and leadership as required.

The successful candidate will demonstrate clinical reasoning, critical thinking and will be key to supporting clinical work across the healthcare team to meet the needs of patients. You will participate in and lead change for the benefit of patients and/or the organisation and be a key member of the senior clinical team.

Job responsibilities

Clinical Practice

  • Assess and holistically clinically examine patients with both minor and complex needs through history taking, physical examination and appropriate use of investigations to confirm diagnosis.
  • Plan, implement and evaluate an effective management plan for the patient, ensuring safe discharge/referral and/or transfer to an appropriate health care service.
  • Provide information and advice on prescribed or over-the-counter medication on medication regimens, side-effects and interactions.
  • Prioritise health problems and intervene appropriately to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care.
  • Refer and discharge safely ensuring that safety-netting advice is offered.
  • Triage telephone referrals, calls from external clinicians, patients and others into a range of services appropriately as required.

Communication

  • Utilise and demonstrate sensitive communication styles, to ensure patients are fully informed and consent to treatment.
  • Communicate effectively with patients and carers, recognising the need for alternative methods of communication to overcome different levels of understanding, cultural background and preferred ways of communicating.
  • Anticipate barriers to communication and take action to improve communication.
  • Maintain effective communication within the organisation and with external stakeholders.
  • Ensure awareness of sources of support and guidance and provide information in an acceptable format to all patients, recognising any difficulties and referring where appropriate.

Non-Medical Prescribing

  • If you are employed in a prescribing capacity, you must only prescribe within your scope of practice and comply with the gtd healthcare NMP governance registration processes.
  • You must maintain your knowledge and skills and prescribe within your scope of clinical competencies.
  • The non-medical prescriber is required to comply with organisational NMP governance processes and adhere to local and national prescribing guidance.

Delivering a Quality Service

  • Recognise and work within own competence and professional code of conduct as regulated by the NMC/HPC etc. Understand own role and scope and identify how this may develop over time.
  • Produce accurate, contemporaneous and complete records of patient consultation, consistent with legislation, policies and procedures.
  • Prioritise, organise and manage own workload in a manner that maintains and promotes quality.
  • Ensure professional revalidation requirements are met.
  • Deliver care according to NSF, NICE guidelines and evidence-based care.
  • Assess effectiveness of care delivery through self and peer review, benchmarking and formal evaluation.
  • Initiate and participate in the maintenance of quality governance systems and processes across the organisation and its activities.
  • In partnership with other clinical teams, collaborate on improving the quality of health care responding to local and national policies and initiatives as appropriate.

  • Understand and apply guidance that supports the identification of vulnerable and abused children and adults, and be aware of statutory child/vulnerable patient health procedures and local guidance.

Desirable

  • Mentor/Teaching qualification. Clinical supervision training and experience.
